ESSENTIAL DUTIES AND RESPONSIBILITIES: Understand a client’s care plan and adequately implement it. Assist clients according to their care plan which may include: companionship, meal preparation and service, light housekeeping, transportation, shopping, medication reminders, bathing, body care, dressing, feeding, ambulation, positioning, transferring, toileting, and protective supervision. Notify the Care Manager of client concerns, changes in care or client condition and client emergencies. Notify the Care Manager and/or Staffing Coordinator of any safety concerns. Follow company Infection Control practices; notify the office if in need of appropriate Personal Protective Equipment (PPE) or supplies. Follow the ClearCare Telephony Instructions to clock in and clock out for all scheduled shifts and accurately update your task status for each client prior to clocking out and leaving the client’s home. For compensable worked hours that are not allocated to a specific client (such as training time), you must use and submit the Time Log and Time Certification for Non-Visit Time form for each period pay period. Conform to the company dress code as specified in the Employee Handbook. Report to work on-time and ready for the day. Complete ongoing training as required by client care needs and/or changes in company policies and procedures. Attend staff meetings. Adhere to all company policies and procedures as detailed in the Employee Handbook. ORGANIZATIONAL RELATIONSHIP: The caregiver is supervised by the Care Manager and receives scheduling direction from the Staffing Coordinator. PHYSICAL AND ENVIROMENTAL DEMANDS: Reposition and transfer clients, sustain weight of clients using proper transfer safety techniques and equipment (example: use of transfer belt while assisting client to transfer from bed to chair), and travel to client’s residence place of residence using a reliable means of transportation. Complete required assessments upon the customer’s admission, transfer, and/or discharge. Receive telephone orders from physicians and record on the Physicians’ Order Form and P.O.S. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered. Take and record TPR’s, blood pressures, accuchecks, etc., as necessary. Contact the customer’s physician for: Nursing assessment of change of condition. Medication orders or charges. To inform of incidents. To report abnormal results of Lab and X-ray. Anything else pertinent to the customers’ care and well-being. Ensure the nurses’ notes and monthly summaries reflect the care plan and is being followed. Attend staff meetings, etc., as directed. Perform other related duties as assigned.
