Summary The Oncology Nurse Navigator 2 is a registered nurse (RN) who works with physician practices and multi-disciplinary teams to promote the effective utilization of services and coordination of care for patients within the Cancer Center service line. The Oncology Nurse Navigator 2 contributes to the assessment of patients' psychosocial, financial and clinical needs throughout the continuum of care ensuring high quality care while supporting quality metrics of the Cancer Center and Rush University Medical Center The Oncology Nurse Navigator 2 functions as a liaison between patients, physician practices, the hospital, and the community performing functions that meet the core competencies of oncology navigation; Care Coordination, Communication, Education, and Professional Role/Care as well as the domains of the Profession Practice Model; Relationships and Caring, Evidence Based Practice, Technical Expertise, Critical Thinking and Leadership. The Oncology Nurse Navigator 2 care may be focused in medical oncology, hematology, stem cell transplant, gyne-oncology, surgery, radiation oncology, cancer screening or supporting multi-disciplinary clinics. Other Information Required Job Qualifications: Current license and credentials by the state they are assigned. At least one year of nursing experience required. Earned Nursing Degree at the Baccalaureate level or higher for those hired after January 1, 2014. All transfers to Rush University Medical Center or Rush University Medical Group from other Rush entities must have an earned nursing degree at the baccalaureate level or higher prior to the transfer. Chief Nursing Officer must provide approval for any RNs with ADN degree hired after 10/22/2020, based upon unit need. ADN RNs hired after 10/22/2020 must show continued enrolment towards a BSN degree with a signature of contract stipulating a BSN degree must be completed within 3 years of hire, Failure to do so will result in termination. ADN RNS are not eligible for leveling to RN3. Current CPR Certification. Analytical ability. Communication skills to lead and teach. The ability to check documents for errors, use a keyboard to enter or retrieve data, and closely examine specimens, images, or reports. A specialty certification may be required for certain departments within 12 months of hire. Preferred Job Qualifications National Certification. Physical Demands Ability to walk throughout the Medical Center and to be standing or walking most of the designated shift. Ability to lift or carry objects 35-40lbs. and lifting and positioning patients, some of whom may exceed 300 lbs. Work conditions include performing procedures where carelessness could result in injury or illness and/or contact with potentially infectious materials and/or strong chemical agents. Technical Expertise Responds to patient questions and needs via various technology: “My Chart,” staff messages, telephone contact, etc. Coordinates/Schedules admissions, surgeries, laboratory testing, radiographic exams, and infusion. Performs basic administrative tasks related to role Establishes a plan for venous access via assessment of peripheral veins. Performs nursing procedures per policy. Assists providers with procedures utilizing appropriate technique. Assesses patient and family learning style to provide ongoing education regarding diagnosis, staging, treatment, side effects, and self-care. Provides detailed documentation in the electronic medical record to support assessment and interventions Critical Thinking Reviews medical records to ensure completeness as applicable for patient diagnosis and staging. Requests additional medical records as needed Ensures new patients are scheduled for appropriate providers based upon assessment of patient and collaborates to alter plan as needed Assesses patients in person or remotely to evaluate current status and needs Provides telephone triage as defined by guidelines and in collaboration with provider coordinating care with infusion room, home health, urgent care, or emergency department as required Recognizes changes in patient status and responds promptly to optimize patient outcomes Prioritizes complex patient care based upon knowledge and clinical experience. Assesses patient compliance with treatment plan Leadership Demonstrates accountability for professional practice, continuing education, and progress toward achievement of goals Establishes and maintains expertise regarding disease, staging, current treatment options, and management of adverse events. Serves as a liaison between practice and referral sources Participates in quality metric initiatives supporting optimal outcomes and value-based metrics Assists in patient identification for tumor board, coordinates tumor board, and participates in patient presentation and care plan development.
