Job Description JOB SUMMARY Under the supervision of the UCSF Health Primary Care Administrative Nurse Manager, guided by policies and procedures of the Ambulatory Services, the Hospital Assistant III assists in the delivery of care to patients at UCSF Primary Care. Hospital Assistant III will be primarily responsible for the members of their primary care team as well as providing back up support to other teammates. The Hospital Assistant III will play a critical role in the patient experience including optimizing clinic flow, providing in and outreach to patients and working collaboratively with the team to create a patient centered environment. This position floats between the front and the back office performing both clerical and clinical duties. Work includes a variety of personal care and nursing care procedures including assuring positive care outcomes. S/he will perform assigned patient care and laboratory procedures in support of services provided to patients within the practices. Certification in phlebotomy (CPT) Demonstrated computer familiarity, keyboard skills, computer literate in programs such as Microsoft Word and Excel. Job Description The University of California, San Francisco Heart & Vascular Center’s Advanced Heart Failure Comprehensive Care Center is seeking a full-time Advanced Practice Provider (APP) for a night-shift position to care for patients with advanced heart failure, mechanical circulatory support (MCS), heart transplantation, and other complex cardiovascular conditions. The APP will join a highly experienced multidisciplinary team of physicians, surgeons, APPs, nurses, pharmacists, social workers, and researchers who provide comprehensive care for advanced heart failure patients across the continuum of care. UCSF’s Advanced Heart Failure and MCS programs are nationally recognized for delivering cutting-edge therapies and support in both inpatient and outpatient settings, including a wide range of temporary and durable mechanical circulatory support options. Serving as a primary provider in the hospital setting, the APP will collaborate closely with Advanced Heart Failure Cardiologists, Cardiothoracic Surgery and Intensive Care teams, as well as nursing, case management, social work, and other multidisciplinary partners to evaluate, treat, and manage this highly complex patient population in both Intensive Care and Acute Care Units. This position is primarily night-based and offers the opportunity to practice at the forefront of advanced heart failure, transplant, and MCS care within a leading academic medical center. Job Description Under the supervision of the UCSF Department of Surgery & Department of Medicine and guided by the policies and procedures, rules and regulations, and bylaws of the UCSF Medical Center, some Nursing Policies, and, Medical Staff Policies, and any unique to inpatient or ambulatory patient services (for those APPs providing services both in the inpatient and ambulatory environments) this Nurse Practitioner (NP) position is assigned to the Berkeley Cancer Outpatient Practice and Infusion Center. This APP position may perform delegated functions as appropriate to their “scope of practice”. In general, the “scope of practice” includes the management of chronic stable medical conditions, specialty protocols (e.g. chemotherapy), routine care, acute or critical care, and medical and surgical procedures. This position will provide outpatient care in approved age groups at UCSF Health settings. The APP has responsibility for the quality of standards of care for a specified patient population and evidences self-direction and accountability in the development of this role within the all UCSF medical center locations. UCSF Health requires all Advanced Practice Providers (APP) to be credentialed through its Medical Staff Services Office (MSSO) in order to practice and function in their clinical role. This applies to both adult and pediatric APPs in the inpatient and outpatient clinical settings at all UCSF Health sites (Parnassus, Mt. Zion, Satellite Clinics, et al). The Advanced Endoscopy service provides state-of-the-art surgical treatments, including cyst-gastrostomy, endoscopic necrosectomy, endoscopy ultrasound-guided gastrojejunostomy, and endoscopic ultrasound-guided biliary drainage procedures.
