Location New York, New York Shift: Day (United States of America) Description: Care Manager RN Neurology Service Lines - Weill Cornell - Days At NewYork-Presbyterian/Weill Cornell Medical Center, our Registered Nurse Care Managers are essential to delivering coordinated, high-quality care for complex patient populations. The RN Care Manager for the neurology service lines supports a highly vulnerable population requiring intensive, multidisciplinary coordination across the inpatient and outpatient continuum. In this role, you will drive care coordination, promote efficient patient flow, and ensure safe, patient-centered transitions through proactive collaboration with the interdisciplinary team. Key Responsibilities · Serve as a core clinical member of the interdisciplinary team, leading daily interdisciplinary rounds to ensure clear, timely communication of patient care plans, goals, and discharge readiness. · Coordinate comprehensive care for neurology/neurosurgery/spine patients across the continuum, recognizing the complexity and vulnerabilities of this population. · Perform initial and ongoing nursing assessments to evaluate clinical needs, identify risks, and support care planning. · Conduct utilization review to ensure patients meet criteria for the appropriate level of care and support optimal use of acute care resources. · Proactively identify and resolve barriers to patient flow, collaborating with physicians, nursing, social work, pharmacy, and ancillary services. · Develop and implement safe, patient-centered discharge plans, including coordination of post-acute services such as skilled nursing, home infusion (IV therapy), and durable medical equipment (DME). · Act as a liaison between inpatient teams, outpatient providers, and community agencies to ensure continuity of care. · Provide patient and family education related to discharge plans, services, and ongoing care needs. Preferred Criteria Case Management Certification (CCM) or willingness to obtain Master's Degree in healthcare related field Minimum of 2–3 years of acute care nursing experience required; neurology/neurosurgery preferred Prior experience in care management, utilization review, or discharge planning Required Criteria Bachelors degree in Nursing (BSN) Current NYS Registered Nurse (RN) licensure 3 to 5 years of health care experience in an acute care setting Strong clinical assessment, critical thinking, communication, and organizational skills with the ability to manage complex patient needs. Job Responsibility Manages and leads team members, operations and patient care activities within a patient care unit during assigned shift. Collaborates with team members and nursing management in the development of methods, techniques and analytical approach to patient care. Communicates, builds consensus, and guides team members toward achieving department objectives. Promotes and maintains high quality patient and family centered care in a healing environment. Creates a professional work environment that encourages team member respect, engagement, accountability and optimal patient outcomes. Develops and implements a nursing plan of care and makes recommendations for care based on patients' needs. Supervises, trains and evaluates home health aides, homemakers and personal care workers in patients' homes. Requests written orders from physicians for changes in the patient plan of care as needed. The schedule includes functioning in a backup On Call capacity 2-3 times per month and 2-3 holidays per year.
