Additionally, the RN is involved in symptom management, patient and family education, and coordinating care within the interdisciplinary team. Inpatient Oncology Float Pool Shift: Nights Schedule: 36 hours per week, with rotating weekends Primary Duties And Responsibilities This position is eligible for full time benefits Deliver direct oncology nursing care, including comprehensive assessment, individualized care planning, symptom management, and evaluation of patient outcomes for adult oncology patients and families. Administer antineoplastic agents and supportive therapies per protocols and ONS standards; verify orders, complete independent double checks, and monitor for and manage infusion reactions and adverse events. Manage central venous access devices (e.g., ports, PICCs); perform access, maintenance, blood draws, and blood product transfusions per policy; ensure safe use of infusion equipment and PPE. Provide patient and family education on diagnosis, treatment plans, self-care, side-effect management, and discharge instructions; coordinate multidisciplinary resources and community referrals. Document care in Epic in accordance with regulatory and institutional standards; maintain accurate medication administration records, flowsheets, and patient education documentation. Participate in quality and safety activities (e.g., safety huddles, infection prevention, hazardous drug handling, Joint Commission readiness) and support clinical research protocols as applicable (e.g., protocol adherence, specimen handling, adverse event reporting). Collaborate with physicians, advanced practice providers, pharmacists, social workers, and research staff; contribute to effective team communication, handoffs, and patient throughput. May serve as charge nurse, preceptor, or mentor; provides functional direction to licensed and unlicensed support personnel in accordance with scope of practice and institutional policies. Knowledge, Skills And Abilities Proficiency in chemotherapy/immunotherapy administration per ONS guidelines; working knowledge of oncology pharmacology and side-effect profiles. Competence in central line care and infusion therapy; ability to administer and monitor blood products per policy. Ability to recognize and respond to oncologic emergencies (e.g., infusion reactions, febrile neutropenia, tumor lysis syndrome) and escalate care appropriately. Strong patient education skills with ability to tailor communication to diverse populations and varying health literacy levels; commitment to culturally competent, trauma-informed care. Proficiency with EMR systems (Epic preferred); accurate and timely clinical documentation and electronic medication administration. Knowledge of infection prevention, hazardous drug handling (USP ), PPE requirements, and universal precautions. Effective triage, prioritization, and time management skills for varying acuity in a fast-paced ambulatory environment. Collaborative team skills; ability to work across interdisciplinary teams and support clinical research activities. A Bachelor’s degree in nursing is strongly preferred, and candidates with an associate degree must complete a BSN within 4 years. 1 year of recent experience as a Registered Nurse preferred. Acute care nursing experience preferred; oncology/hematology experience in an Inpatient or ambulatory infusion clinic setting preferred. Experience with electronic medical records (e.g., Epic) preferred. License/Certification/Registration Required Current unrestricted Massachusetts Registered Nurse (RN) license required. ONS/ONCC Chemotherapy/Immunotherapy Certificate required or obtained within 6 months of hire (role-dependent). Oncology Certified Nurse (OCN) preferred. Basic Life Support (BLS) required; Advanced Cardiovascular Life Support (ACLS) preferred depending on work area. Supervisory Responsibilities No Patient Contact Direct and continuous contact with adult oncology patients and families in an inpatient environment. As an integral member of the Population Health Management Operations team, The Transition of Care Nurse Care Manager provides episodic care management for Medicare Shared Savings Program (MSSP) and Medicaid ACO patients from inpatient admission to home. This includes follow up phone calls after discharge, medication reconciliation, and ensuring patient has appropriate follow up appointments scheduled as well as documenting and ensuring all billing requirements are met prior to submitting TCM charges. The Transition of Care Nurse Manager directly interfaces with patients (via phone), Primary Care physicians, pharmacists, care managers, and health care teams involved in patient care as well collaborating with PHM Clinical Operations leadership. Primary Responsibilities: 1. Manages episodic transitions of care for Medicare Risk and Medicaid risk patients from inpatient discharge to home as applicable. a. Calls all discharged patients within two business days of discharge and conducts post discharge assessments. b. Works alongside the PHM Clinical Pharmacist to identify and perform Medication Reconciliation for identified patients within the two day follow up phone call. c. Reviews discharge instructions/paperwork prior to call to review with patient any action items needed prior to follow up appointment. d. Facilitate face to face follow-up appointment with their PCP within 7-14 days after discharge (or according to discharge instructions if applicable). e. Document patient interaction (phone calls) in the appropriate patient chart in the electronic medical record. f. Manages and coordinates transitions of care by communicating the care plan to other providers and care managers and applicable practice staff. g. Maintains all documentation according to standards and requirements. h. Ensures all Transitions of Care (TCMs) meet appropriate billing requirements prior to submitting documentation to the billing department per protocol yet TBD. 2. Demonstrates effective teamwork and collaboration with the primary care provider and the care team a. Engages the patient and caregiver as active members of the care team and facilitates an organized and effective, warm hand off for transitions of care back to the patient's medical home (PCP). b. Responsibilities We are actively hiring for an Registered Nurse (RN) Endoscopy. Develop an individualized perioperative patient plan of care by implementing: P rotecting the patient from injury caused by extraneous objects and chemical, electrical, mechanical, and thermal sources. D emonstrating safe operation of equipment and machinery and follows procedures for reporting and correcting an unsafe situation. S peak ing up with safety concerns acting as the patients advocate. Handling specimens according to facility policy and procedures. Adher ing to standard precautions including the use of personal protective equipment. Complying with Universal Protocol and facility site validation and time-out policy C ommunicating the patient’s current status to relevant parties throughout the continuum of care. A dministering medications safely and correctly using the 8 rights of medication administration. P racticing and monitoring aseptic technique . Implement ing the plan of care including the following interventions: M anaging patient recovery from anesthesia – Moderate sedation and Monitored anesthesia care . Assess ing patient status at intervals in accordance with facility policies and procedures . P roviding nursing care and appropriate monitoring in accordance with policies and procedures . P erforming interventions to maintain the patient’s tissue perfusion at or above baseline levels. Ensuring scopes are prepared for use in accordance with policies and procedures and manufacturer’s instructions for use . Maintain ing endoscopy instruments and accessories in working order. Troubleshooting and recognizing malfunctions . P erforming interventions to maintain the status of the patient’s genitourinary, gastrointestinal, musculoskeletal, endocrine, respiratory, cardiovascular, and neurological systems and fluid, electrolyte, and acid-base balances at or above baseline levels. P erforming interventions to ensure the patient is at or returned to normothermia at the conclusion of the immediate postoperative period. P erforming interventions to protect the patient from infection. A ssessing the knowledge level of the patient or designated support person. P roviding education regarding the expected psychosocial response, nutritional management, medication management, pain management, and expected responses to the procedure. I nvolving the patient or designated support person actively in decisions affecting his or her perioperative plan of care and the rehabilitation process. P rotecting the patient’s rights, dignity, and privacy. P roviding age-specific, culturally competent, ethical care within legal standards of practice . Evaluate the patient’s progress toward attaining outcomes. Transfer care as applicable. Provide hand-off report to other teammates that are accurate, concise and pertinent to the patient’s condition and response to care. Delegate tasks according to the state Nurse Practice Act (if applicable), state board of nursing or professional registration requirements, state laws, and job descriptions . Document episode of care accurately, timely, completely, and legibly. Demonstrate effective communication and collaboration skills with members of the team and other stakeholders. Qualifications Graduate of an a ccredited s chool of n ursing . O ne year of GI, OR and /or medical / surgical nursing experience preferred . Current state nursing licensure to practice in good standings . Successful completion of Basic Life Support (BLS) or Basic Cardiac Life Support (BCLS) within 90 of employment or documentation of current BLS/BCLS certification. Successful completion of Advanced Cardiovascular Life Support (ACLS) Course within 90 days of employment or documentation of current ACLS certification. Successful completion of Pediatric Advance Life support (PALS) course is required when pediatric care is included in the facility scope of services within 90 days of employment or documentation of current PALS certification.
