Acts as a liaison between clinical staff and management focusing on coordinating the clinical operations of the practice, Panel Management, Patient Education and Health Coaching. In partnership with the practice leadership team, including Clinicians, CBHM, MSW, RN and Practice Manager supports care management efforts within the team through process improvement, workflow redesign, providing assistance with training, and delegating to other members of the team. Position Responsibilities Collaborates with Practice Leadership to conduct Daily Huddles and Weekly Care Team meetings, in which the team will review practice population health data to improve care and achieve practice goals. Prepares exam room as required based on chief complaint.
