Company Allegheny Health Network Job Description GENERAL OVERVIEW This job is an integral part of the care delivery system working together to provide a healing environment, by placing our customers first. The incumbent provides specialty and critical care healthcare services in physician office(s) and non- acute based settings with a focus on chronic health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization within the CRNP scope of practice. Specialty certification, advanced procedures with appropriate privileging, weekend and on-call coverage on a rotational basis, as required. E-consults and telehealth could be included in the care provided. Essential Responsibilities Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, discharge planning and providing accurate medical record documentation for patients in physician office(s) and non- acute based settings. 30% Communicates with attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the CRNP's knowledge base or scope of practice. Makes medical diagnoses and institute appropriate therapy within the applicable scope and standards of practice.15% Orders, interprets, and evaluates laboratory, radiological, and diagnostic studies appropriate for the patient's condition to determine and implement treatment plans.10% Assesses learning needs of patient and family and provides education based on age, culture and willingness to learn.
