About the Role: Work with a dynamic and innovative team to provide personalized care to patients from several area hospitals, including Temple Hospital, Roxborough Hospital, Albert Einstein Medical, and Chestnut Hill Hospital. Provide direct patient care, including assisting with activities of daily living, vital signs, and mobility. Collaborate with our interdisciplinary team to develop and implement individualized care plans. Maintain accurate and timely documentation of patient care. Support a culture of excellence, infection control, and quality improvement initiatives.
