REPORTS TO: Clinical Services Director and RN Essential Functions: The following duties are normal for this position. These are not exclusive or all-inclusive. Other duties may be required and assigned. Assesses resident needs, and provides necessary nursing care as per resident's care plan. Directly supervises CNAs on one's assigned team. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include training employees; planning, assigning, and directing work; appraising performance, addressing complaints and resolving problems. Responsible to participate in Pathways of Purpose program by applying functional engagement during workday in all daily interactions with residents . Ensures that resident rights are maintained Administers prescribed medications and treatments in accordance with approved nursing techniques and per physician orders. Documents nursing history and physical assessment for assigned patients upon admission; Handles resident, family, or physician complaints/concerns, referring issues to CSD as necessary. Respects advance directives and assists residents to a dignified death; and provides emotional and spiritual support to family and other staff as needed. Responds to life threatening situations based upon nursing standards, policies, procedures, and protocol. Participates in department and/or unit performance improvement activities. Other duties as assigned § Attends in-service training programs for department specific and other relevant programs, i.e., safety, infection control, orientation, etc., as scheduled. § Is aware of and carries out general community and specific departmental safety policies; always reports hazards/safety issues promptly. § Is aware of and practices principles of infection control; reports infection control issues promptly. MINIMUM TRAINING AND EXPERIENCE: A current and valid nursing license. A minimum of three years of experience in a senior living community; a minimum of three years of experience in a management/supervisory position or any equivalent combination of training and experience which provides the required skills, knowledge and abilities CERTIFICATION AND CLEARANCE REQUIREMENTS: · First Aid and CPR certification · LifeScan criminal record clearance or criminal record exemption, as required by law · Health evaluation to ensure ability to perform duties of the position and the absence of any communicable disease, including tuberculosis · Basic Class “C” Driver’s License with a clean driving record, and free of restrictions or limitations as required by State regulation IND456 
JOB SUMMARY Assisting the Director of Nursing Services in the planning, organizing developing and directing of the day-to-day functions of the Nursing Department in accordance with current federal, state, and local standards, guidelines and regulations, and facility policies. Operating as a CNA providing direct care to residents, the objective is to ensure the highest degree of quality care is maintained at all times. QUALIFICATIONS Must have current certification as a CNA Must possess current CPR Certificate or become certified within two weeks of employment. Demonstrate interest and/or knowledge in the therapy, rehabilitation, restorative, ADL, splint, and ROM program. Must be able to read, write, and speak the English language in an understandable manner. Must possess the ability to make independent decisions when circumstances warrant such action. Must be in good general health and demonstrate emotional stability. Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people. Must be able to lift, push, pull and move equipment, supplies, and customers in excess of fifty (50) pounds throughout the day. Is subject to exposure to infectious waste, diseases, conditions, etc., including exposure to the (HIV) AIDS and Hepatitis B viruses. ESSENTIAL FUNCTIONS Review the departments Policy & Procedure Manual, Job Descriptions, cleaning schedules, etc., at least annually for revisions and make recommendations to the Director of Nursing. Collaborates with CNAs to ensure compliance with established departmental policies and procedures, including appropriate dress code. Assist in developing a written plan of care for each customer that identifies the problems/needs of the customer. Attends and actively participates in customer care plan conference when indicated Resolves then processes a concern resolution form on all concerns, complaints made by the customer or their family to the Administrator. Under supervision of a clinical leader, meets with the CNA Ladies and Gentlemen of the nursing department, on a regularly scheduled basis to assist in identifying and correcting problem areas, and/or the improvement of services. Recruit and conducts preliminary interviews and make recommendations for hiring to the clinical leadership and Administrator. Final interviews and approval are completed by the clinical leadership or Administrator. Provides assistance with orientation, coaching and mentoring to ensure the success of CNAs in the performance of their duties. Makes recommendations for counseling to the clinical leadership. Provides feedback to clinical leadership regarding CNA daily and annual performance evaluation feedback. Make daily rounds to ensure that nursing personnel are performing required duties, and to ensure appropriate procedures are being followed. Make physical rounds of all customers daily. Make rounds with physicians as necessary. Maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department. Plan, conduct and participate in In-service Education Programs. Assist in implementing and maintaining an effective orientation program that orients the new CNA Ladies and Gentlemen of Alden to the facility, his/her department, and its policies. Attend and participate in continuing education programs. Recommend to the Director of Nursing the equipment and supply needs of the department. Place orders for equipment and supplies as necessary following established budgetary guidelines and assists with the distribution of supplies to ensure that the CNAs have all necessary equipment and supplies. Assist in developing methods for coordinating nursing services with other customer services to ensure the continuity of the customers total care regimen. Monitor and perform administrative duties such as completing forms, reports, studies, charting, etc., as necessary. Monitor absenteeism to ensure that an adequate number of nursing care personnel are on duty at all times. Report problem areas to the Director. Assist and maintain general organization of nursing units. Provide the Director of Nursing with information relative to the nursing needs of the customers and the nursing service department’s ability to meet these needs. Assist with arranging appointments and transportation as needed. Assist support services in developing, implementing, and conducting in-service training programs that relate to the nursing services department. Ensure that all personnel involved in providing care to the customer is aware of the care plan and that care plans are used in administering daily care to the customer. Attend Department head meetings, Infection Control meetings, Quality Improvement meetings, etc., as directed. Maintains daily oversight of POC documentation compliance Perform other related duties as assigned. Assists in maintaining Airtable compliance by completing and uploading Core Competency Checklist and submitting the Competency Evaluation via digital Airtable form. Prepare patients for examination, which includes conducting interviews to verify patient information, record medical history; confirm purpose of visit; etc. Perform preliminary physical test (blood pressure, weight, temperature, etc.) for vital signs and escalate critical cases to provider as needed. Conduct point of care testing as needed (i.e., flu, strep, urinalysis, ECG, HCG); and provide patient information to provider. Responsible for stocking supplies and maintenance of exam rooms, front desk, and reception area; ensure providers have medical supplies needed. Responsible for completing order sheets to maintain supply inventory levels; regularly check expiration dates on supplies. Responsible for maintenance of medical records and scanning charts to patient files. Answer phone calls, collect and sort daily incoming and outgoing postal correspondence. Ensure reception area is well maintained. Conduct courtesy call-backs for patient follow-up. Follow operating instructions to perform and document daily controls and calibration of equipment; maintain Quality Assurance/Quality Control logs for equipment troubleshoot breakdowns, perform preventive maintenance, and submit repair tickets as needed.
