This is a full time, night shift opportunity. 6 Cathcart is a complex 43 bed medical-surgical unit at Pennsylvania Hospital that provides the highest quality of care to its patients. 6 Cathcart also achieves outcomes that exceed national benchmarks in quality, employee engagement and patient satisfaction through a strong interdisciplinary team approach. Surgical service lines include Colorectal, Urology, Gynecology, Otorhinolaryngology and General Surgery. While a majority of our patient volume is surgery, we also care for general medicine patients. RN patient assignments are most frequently 3-5:1, depending on the overall acuity of the assignment, which is measured by our acuity scale. These patient assignments are also supported by highly skilled Patient Care Technicians (PCT) that perform vital signs, accuchecks, electrocardiograms, intake and outputs, phlebotomy, assistance with ADLs and ambulation. The PCT patient assignments are most frequently 8-12:1, again depending on the overall acuity of the assignment. The Clinical Nurse 1 New to Practice/Apprentice is accountable for the provision of direct care to patients who may range in age from infancy to the elderly and demonstrates within the competency domains of Continuous Quality Improvement, Evidence Based Practice and Research, Leadership, Person and Family Centered Care, Professionalism, Safety, Teamwork, and Technology/ Informatics (Definitions listed below). The Clinical Nurse 1 New to Practice/Apprentice works closely with the interprofessional health care team to facilitate the coordination of care across the continuum. The nurse focuses on developing the knowledge and skills necessary to provide individualized care based on physical, psychosocial, cultural, educational, safety, and age-appropriate considerations of assigned patients. The Clinical Nurse 1 New to Practice/Apprentice requires consultation with more experienced clinicians and benefits from feedback while demonstrating growth in the ability to care for increasingly complex patients. The nurse demonstrates how nursing practice impacts the organizational vision, mission, and goals and the care delivery system. The nurse complies with all regulations and standards of regulatory and accrediting bodies. The Clinical Nurse 1 New to Practice /Apprentice must complete all Clinical Nurse 1 competencies and is expected to advance to the Clinical Nurse 2 Colleague level by 18 months of professional practice. Major Duties and Responsibilities Directs the daily activities of the licensed practical/vocational nurse and certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures. Follows the nursing model to promote holistic care for the residents. Ensures that policies and procedures are complied with by nursing personnel assigned. Participates in the admission, discharge and transfers of residents as required. Assesses for changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly. Transcribes physician orders to medical record and carries out orders as written. Prepares and administers medications as per physicians’ orders and observes for adverse effects as indicated. Performs wound treatments as per physicians’ orders, observes for changes and documents accordingly as indicated. Schedules follow up appointments for residents and transportation needs as indicated. Provides nursing leadership to nursing personnel assigned to the unit. Provides direct care skills such as colostomy changes, tube feedings, wound care, suctioning, IV administration, etc. in accordance with current policies and procedures, as assigned. Performs rounds to ensure resident needs are being met and personnel are performing their assigned duties. Collaborates with other members of the interdisciplinary team as needed to ensure residents’ needs are holistically met. Participates in management team meetings as indicated to discuss census changes, resident changes in status or care need, or complaints or concerns. May assist in evaluating work performance of nursing personnel and implements discipline according to operational policies and as directed. Informs appropriate departments of concerns or complaints and files grievances as necessary. Reports any incidents or unusual occurrences to the unit manager, assistant director or nursing or director of nursing and participates in the investigative process as needed. Participates in end of life care as required. Ensures that there is adequate stock of medications, supplies, equipment and notifies appropriate personnel of needs. Initiates, reviews and updates care plans as required. Promotes teamwork, mutual respect, and effective communication. Participates in the survey process when inspection surveys arise. May participate in QAPI activities as needed or assigned. Ensures that infection control principles are followed and helps correct deficiencies in practice as noted. Other duties as assigned Experience and Education :Required Qualifications Prior experience in long-term care preferred but not required. A Nursing degree from an accredited college or university. Current unrestricted license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in practicing state. Current CPR certification
