Registered Nurse – Intensive Mobile Treatment Team (Full Time) About the Program Intensive Mobile Treatment (IMT) is a trauma-informed, community-based behavioral health program that provides treatment, rehabilitation, and support services to individuals who have not been successfully served by traditional systems. IMT teams deliver care directly in the community, including shelters, public spaces, and other natural settings. Position Overview The Registered Nurse (RN) serves as a primary practitioner responsible for assessing and monitoring participants’ clinical health and response to treatment. The RN works collaboratively with a multidisciplinary team including psychiatry, social work, peer specialists, and other staff to support participants in achieving their mental health and recovery goals. Key Responsibilities · Conduct psychiatric and physical health assessments for program participants. · Develop and coordinate care plans with the multidisciplinary treatment team. · Monitor participant health status and modify treatment plans as needed. · Administer medications, injections, vaccines, and perform phlebotomy as ordered. · Monitor vital signs and medication side effects and communicate findings to the treatment team. · Coordinate medical appointments and collaborate with psychiatrists and community physicians. · Provide treatment, rehabilitation, and recovery support services. · Advocate for participants and assist them in accessing community resources and services. · Maintain accurate clinical documentation, progress notes, and medical records. · Participate in team meetings, supervision, and required trainings. · Provide crisis intervention services on a rotating basis.
