Primary Care Provider WelbeHealth PACE (Program of All-Inclusive Care for the Elderly) provides seniors with the opportunity to continue living in their homes and in their communities. MAJOR RESPONSIBILITIES/ESSENTIAL FUNCTIONS Position Summary Reporting to the Manager, with direction from the Registered Nurse, this position is responsible for providing high quality, individualized basic care activities as identified by the RN, using the components of the nursing process.
