Case Manager, RN - Care Coordination

Case Manager, RN - Care Coordination 1.0 FTE, 40hrs per week, 8hr shifts. Start time 630AM, 7AM or 730AM. Has responsibility, in collaboration with the health care team and family, for the coordination of care for specific patient populations from admission through discharge. This includes advocacy, accountability for the achievement of identified outcomes in a timely manner, promotion of collaborative practice, appropriate use of resources, facilitation of timely discharge, coordination of discharge planning, resource acquisition, and patient/family education. This is accomplished through initial and ongoing assessment, effective participation in the care planning process, and collaboration with the team and community providers. Supports staff/physician education related to the care coordination process. Required Education and Experience BSN degree or equivalent combination of education and experience. As an integral member of the Health Care Team, the RN functions within the Scope of Practice as defined by the State, the ANA Scope and Standards for Pediatric Nursing Practice, and adheres to Seattle Children's policies/procedures and guidelines of care. Responsible for providing safe, family centered patient care. Using the Nursing Care Process, the RN is responsible for assessing, planning, implementing and evaluating patient care in a collaborative method with the patient and patient's family, significant others, and members of the health care team. May be required to provide supervision and delegation of tasks to Nurse Technicians, Licensed Practical Nurses, Certified Medical Assistants, Certified Nursing Assistants, and others as defined by the State Administrative Code. This CICU is a 24-bed unit quaternary care referral center for four states: Washington, Alaska, Montana, and Idaho (WAMI region). Patients cared for are newborns up to young adults with acquired and congenital heart disease. The CICU supports robust mechanical circulatory assist and cardiac transplant programs. Specialized care provided in the CICU includes cerebral oximetry monitoring, inhaled nitric oxide, temporary pacemakers, heart failure management, open chest procedures, and continuous renal replacement therapy (CRRT). Extracorporeal life support represents a highly specialized form of cardiopulmonary support that includes the use of ECMO and ventricular assist devices (VAD). As an integral member of the Health Care Team, the RN functions within the Scope of Practice as defined by the State, the ANA Scope and Standards for Pediatric Nursing Practice, and adheres to Seattle Children's policies/procedures and guidelines of care. Responsible for providing safe, family centered patient care. This role provides patient centric care and is primarily responsible for nursing assessment and diagnosis, planning, implementation, and evaluation.
