All affiliated Home care aides employed by Nurse Next Door to provide in-home care services will be 18 years old (or if under 18 years of age be a high school graduate or enrolled in a vocational education program) and have sufficient communication and language skills to enable them to perform their duties and interact effectively with clients and other agency staff. Home care aides must meet all registration, criminal background checks and training requirements as specified by the State of California Department of Social Services Home Care Services Bureau and Nurse Next Door policies. Essential Job Functions/Responsibilities Responsibilities of the home care aide include, but are not limited to, the following- · Execute the client’s care plan as outlined by the Care Designer. · Help clients with their ADLs (Activities of Daily Living) that are within the scope of practice for a Registered Health Care Aide. · Assisting clients with personal hygiene such as skin, mouth, hair care, bathing and dressing. · Assisting clients in and out of bed, transfers, positioning and assisting with ambulation. · Assisting with prescribed exercises which clients and aides have been taught by appropriate health personnel. · Meal planning and preparation, including therapeutic diets, and assisting clients with eating. · Assisting clients to the bathroom or in using commodes, bedpans or urinals and incontinence care. · Performing household services including housekeeping, laundry, transportation, correspondence, making telephone calls, shopping for personal care items or groceries, and companionship. · Assisting clients with medications reminders which are ordinarily self-administered. · Reporting changes in the client's condition and needs to the administrator or care designer. · Completing records regarding services performed. · Driving to appointments or to run errands. · Accurately records all hours worked. · Remains alert throughout the shift in case client requires assistance, responds immediately to any such needs by providing the assistance required, or calling the Company for guidance. · Participating in all required entry-level and annual training requirements. · Maintaining an active home care aide registration with the State of Ca. Completes an initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). Provides professional nursing care by utilizing all elements of nursing process. Assesses and evaluates patient’s status by: Writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs, participating in revising the plan of care as necessary. Uses health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. Acts as Case Manager when assigned by Clinical Supervisor/Nursing Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Completes, maintains, and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. Communicates with community health related persons to coordinate the care plan. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. Provides and maintains a safe environment for the patient. Assists the patient and family/caregiver and other team members in providing continuity of care. Works in cooperation with the family/caregiver and hospice Interdisciplinary Group Members to meet the emotional needs of the patient and family/caregiver. Attends interdisciplinary group meetings. Position Qualifications Registered Nurse with valid CA state license. Hospice experience preferred. At least one year of recent hospice experience along with a strong foundation of acute care experience. Maintains a current CPR certification. Valid driver’s license, auto insurance, reliable transportation, and willingness to drive to patient locations.
