As a Hospital Medicine Provider at SSH, you will report, through the Hospitalist Program leadership team including the Lead Advanced Practice Clinician and Program Director. You will have such duties and responsibilities as are set forth below and as may be reasonably requested by SSH from time to time. You shall perform the medical services faithfully, with due diligence and efficiency, to the best of your ability, and in accordance with the applicable standards of your profession, canons of professional ethics and all laws and SSH policies in effect from time to time. SSH recognizes that teamwork and flexibility are essential to providing 24-hour per day patient-centered care. SSH expects you to be respectful of your patients, other health care providers, and the physicians and staff of SSH. You shall be professional at all times and adhere to HIPAA standards with regard to maintaining confidentiality of patient information. You will have several different roles as a SSH Hospital Medicine Provider. These roles include admitting, rounding, and performing consults at the request of other providers. Regardless of role, you will be required to use approved SSH patient care treatment plans and order sets, when appropriate. Compensation Pay Range: $123,947.20 - $224,122.21 Essential Functions: Obtains a detailed and accurate medical history and performs a comprehensive physical exam for patients admitted to the Hospital. Orders necessary diagnostic tests, consultations and procedures as indicated or requested by the Hospital Medicine Consultant or attending physician. Identifies and manages chronic medical conditions and provides continuous care to hospitalized inpatients. Has familiarity with current guidelines regarding the assessment of the risk of a perioperative cardiovascular event for pre-operative patients (such as RCRI, and ACS-NSQIP) and pre-operative medical optimization. Prescribes medications, as necessary. Writes orders in accordance with current practice guidelines. Writes complete and comprehensive notes on all patients using appropriate electronic systems consistent with Hospital policy. Facilitates timely care, as necessary. Actively participates in the South Shore Health System readmission reduction efforts. Reviews the care of each patient with the attending physician and other consultative services. Maintains communications with referring physicians. Collaborates with the patient care team (physicians, nurses, PT/OT, case management) to develop and facilitate care plan. Proactively manages patients to prevent complications. Provides urgent evaluation to patients with changes in clinical status. Prepares patients for discharge, including patient and family discharge teaching, continuing care and rehabilitation arrangements in conjunction with Nursing and Care Progression, writing discharge prescriptions, and confirming primary care or referring physician follow-up. Prepares formal discharge summaries for or with Hospitalist. Possess the maturity, self-confidence, and ability to follow complex patient management plans as directed by the attending physician. Eligible for state and federally controlled substances registration. Demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her assigned unit. Time Management: Exceptional organizational skills and ability to organize time and priorities effectively, asking for direction when appropriate. Flexibility to handle multiple tasks and deadline pressures. Quality Improvement: Skills to work efficiently and effectively and strive to do so in all aspects of position. Minimum Education - This position is a licensed independent practitioner credentialed through the Medical Staff Services Office Completion of Advanced Physician Assistant or Nurse Practitioner training program from an accredited institution. Alongside medical providers, case managers, social workers, behavioral health team members and other HIV nurses, you will provide high quality nursing care to patients consistent with the philosophy of patient centeredness, non-judgement, harm reduction, and team-based care. You will serve in a key clinical role to the care team with primary responsibilities of HIV and other chronic disease management, care coordination, advice/triage needs for the team’s patients, and ensures excellent communication of the care plan across team members. In this role, you will conduct clinical nursing assessments and triage for team patients during routine and urgent medical visits in the outpatient clinic. You will also provide care coordination, medication adherence support, health education, and behavioral risk reduction counseling for HIV primary care patients. This role offers a unique opportunity to promote harm reduction, linkage, and retention to HIV primary care, with the aim of supporting people experiencing homelessness and living with HIV to achieve and sustain health and wellness. This position is funded until the end of March 2028 with the possibility for renewal. If this funding is not renewed, this position will end March 31, 2028. Responsibilities: Provides outpatient primary care nursing including medical assessments and triage services for HIV team patients coming in for same-day or scheduled visits; conducts virtual, telephonic, or in-person nurse assessment and triage to assess the severity of the patient’s health concerns using approved protocols and resources to advise appropriately. Documents patient information accurately and in a timely manner in the EHR. Implements the patient-centered plan of care, evaluates outcomes, and regularly communicates the plan of care, patient status, and progress toward goals with the patient and care team members; conducts intake assessments for new HIV team patients and facilitates rapid linkages to medical providers. Administers vaccines, injectable medications (including long-acting injectable medications for HIV), inhaled respiratory therapies (e.g. nebulized therapies) and provides wound assessment and wound care as indicated; provides medication adherence assessments and counseling, and when directed, delivers directly observed therapy. Provides chronic disease management support for HIV as well as other common co-morbid medical conditions such as diabetes and hypertension. Identifies any structural barriers patients face in accessing medical care and adhering to HIV medications and works with the interdisciplinary team to address those barriers (e.g., housing, transportation, drug treatment, food, clothing, benefits, etc.). Coordinates care with integrated HIV team behavioral health staff. Assists medical providers in coordinating follow up related to the provision of medication (such as buprenorphine and naltrexone) for opioid use disorder and other substance use disorders. Coordinates preventative services for HIV team patients including, but not limited to, education, preventative medications, vaccinations, testing, and referrals. Facilitate patient Linkages with internal teams including office-based addition treatment team, harm reduction team, and medical respite team, as well as with external programs to optimize patient outcomes. Promotes a compassionate and therapeutic environment that is responsive to the unique patient population and extends to members of the care team; demonstrates initiative and commitment to continuously improve services and processes that positively impact patient care and organizational goals. Participates in training and orientation of new nursing staff as directed by Supervisors. Exhibits a positive attitude with patients and establishes professional and respectful relationships with the internal and external healthcare team members, agencies, and healthcare facilities. Participates in training and orientation of new nursing staff as directed by Supervisors. Qualifications: Licensed as Registered Nurse in the Commonwealth of Massachusetts. Bachelor’s degree in nursing preferred; 2 years of experience working in an ambulatory/outpatient care environment required. HIV primary care experience is highly preferred; Certification as AIDS Certified Registered Nurse (ACRN) preferred; however, this can be obtained after being hired, and program funding available to support this training . Expertise in substance use disorder and harm reduction principles, strongly preferred. Experience working with people experiencing homelessness is strongly preferred; Mental Health experience, including knowledge of trauma informed care principals, preferred. Experience working in a community health center setting, a plus. Bilingual in Spanish and English preferred; Bilingual in Haitian Creole a plus. Scheduled Weekly Hours: 24 - Performing under the direction of the RN/LPN and as a member of the unit-based patient care team, the Certified Nursing Assistant performs duties of direct and indirect patient care.
