The Discharge RN will meet the needs of the patient, to facilitate timely, and appropriate discharges from the hospital. The Discharge RN will assist in patient advocacy. Completes and facilitates the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation. Teaches patients/families/caregivers about a patient’s medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures. Translates complex medical terminology to ensure complete knowledge. Completes extra discharge documentation, education, and discharge care for special patient populations, to ensure compliance with core measures for discharge. Ensures patients /families/caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc. Coordinates with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.
