Job Description The University of California, San Francisco (UCSF) Advanced Heart Failure Comprehensive Care Center’s Mechanical Circulatory Support (MCS) Program seeks a full-time ECMO Specialist Nurse to help grow our program. The AHFCCC team consists of highly experienced physicians, surgeons, APPs, nurses, social workers, researchers and others. Our MCS program is nationally known for cutting edge MCS support in both the outpatient and inpatient setting and offers a variety of support systems (long-term and short-term). The ECMO Nurse Specialist is a highly skilled critical care nurse responsible for the initiation, management, and monitoring of Extracorporeal Membrane Oxygenation (ECMO) support for critically ill patients. Working as part of a multidisciplinary team, the ECMO Specialist ensures safe, effective circuit operation, timely response to complications, and adherence to established clinical protocols in order to optimize patient outcomes. The Extra Corporeal Membrane Oxygenation (ECMO) Nurse Specialist will be responsible for providing device related care to patients on ECMO as well as other temporary and durable MCS devices in the inpatient, operative and inter hospital setting. Qualifications REQUIRED QUALIFICATIONS A minimum of four (4) years recent clinical RN experience working in adult critical care setting (ED, ICU). A minimum of two (2) years recent RN experience working with ECMO patients. A minimum of one (1) year experience working with VAD patients. Strong understanding of cardiopulmonary physiology, hemodynamics, and coagulation management. Ability to interpret complex clinical data and respond quickly to changing patient conditions. Excellent critical thinking, problem-solving, and decision-making skills under pressure. ELSO Adult Practitioner Certification (completed or in progress) or successful completion of an ECMO knowledge and skill assessment as part of the interview and selection process. A passing score, based on established criteria will be required for consideration for the position. Strong verbal and written communication skills for interdisciplinary collaboration. Ability to remain calm and function effectively in high-stress, emergent situations. Meticulous attention to detail and commitment to patient safety. Job Description BCH Oakland Day Hospital Infusion Center serves the Specialty populations at UCSF Benioff Children’s Hospital. The treatments include but are not limited to apheresis, chemotherapy, biotherapy, immunotherapy, blood transfusions, hydration and electrolyte therapy, injections and blood draws, enzymes and iron infusions, and antibiotics. The center consists of 14 beds, and treatments include research and standard of care infusions. The infusion centers are staffed by Nurses, Medical Assistants, and a Practice Coordinator. This position requires the incumbent to utilize the nursing process for assessment, planning, implementation and evaluation to provide safe, therapeutic care. Functions as a member of the interdisciplinary and nursing teams; carries out established nursing procedures and performs related duties as required; performs nursing care for patients with complex needs. PEDIATRIC HOSPICE RN CASE MANAGER JOB DESCRIPTION Overview The Hospice Pediatric RN Case Manager will work with our Pediatric team. Primarily responsible for providing professional nursing care to assigned patients. With input from the IDT (interdisciplinary team), patient and family, assesses, develops, implements and evaluates the plan of care. Job Description The University of California, San Francisco (UCSF) Lung Transplant is seeking a full-time Advanced Practice Provider (APP) to join their Advanced Lung Disease Service (ALDS). UCSF is ranked #8 nationally in pulmonology and lung transplant has had significantly higher-than-expected survival rates for post-lung transplant recipients for more than 13 years consecutively! This accomplishment is a result of a great multidisciplinary team that strives to redefine possible at every level of care. The UCSF team consists of highly experienced physicians, surgeons, APPs, nurses, social workers, pharmacists and many others that provide quality care for patients with acute respiratory distress syndrome (ARDS), Alpha-1 antitrypsin deficiency, bronchiectasis, chronic obstructive pulmonary disease (COPD), cystic fibrosis, idiopathic pulmonary fibrosis, interstitial lung disease and pulmonary hypertension. The APP will be responsible for coordinating the care of patients in the hospital setting Responsibilities Work with physicians/surgeons/APP colleagues and other clinical and administrative staff to ensure efficient patient management based on accepted practice, including but not limited to, taking histories, performing physical examinations, peri-operative planning/coordinating with outside providers and primary care providers (PCPs) as applicable, responding to changes in patient status, providing prescriptions/evaluations/referrals, diagnostic tests/treatments as permitted under core privileges and standardized procedures. Evaluate for and order home health services, durable medical equipment (DME), Department of Motor Vehicles (DMV) certifications, refer for consultations, physical therapy, order radiographic imaging or diagnostic tests, and specialized treatments (e.g. chemotherapy with the approved privilege). Provide patient and family education, arrange follow-up appointments, and engage in any approved privileges after the APP has demonstrated competency through proctoring and FPPE process. OMAG will identify independent practice for approved privileges on the ECHO website and procedural activity will be entered by the provider and monitored via the elog system. To evaluate, assess and create a documented plan of treatment per patient’s presenting HPI or established plan of care as defined by the UCSF Medical Staff Bylaws. To answer clinically relevant patient medical advice calls or during bedside communications or in coordination with health care professionals, patients, and their families as necessary. To independently, or in a shared visit model, evaluate routine, acute, critical, new patients, or specialty consultations when needed or required and discuss with attending and staff as appropriate and document in the EHR.
