Registered Nurse (RN) Sub Acute is a licensed professional nurse directly under the supervision of the Director of Nursing Services. The RN Supervisor is to assist the planning, organizing, developing and directing of nursing service to assure that the highest degree of quality resident care can be maintained at all times. Registered Nurse (RN) Sub Acute GENERAL DUTIES AND RESPONSIBILITIES: GENERAL • Assure that residents have a clean, safe, orderly and comfortable environment. • Initiate resident Plans of Care on admission. • Assist in implementing recommendations from consultants and the Quality Assurance team as directed by the Director of Nursing Services. • Assist in developing, reviewing, revising an updating resident Plans of Care for individual residents. • Ensure that notification is given to the resident’s attending/alternate physician, as well as the resident’s legal guardian/representative, when the resident becomes critically ill, injured, etc., or has a significant change in condition as outlined within our established policies. • Review complaints and grievances form families and notify the Director of Nursing Services as necessary. • Evaluate emergencies and assist in implementing emergency measures. • Assure that the unit area is maintained in a clean and safe manner for resident comfort and convenience, by ensuring that necessary equipment and supplies are maintained to perform nursing care. • Assist in notification of physician of any resident incidents/unusual occurrences. Ensure that the Accident/Incident Report forms are filled out and completed on all such occurrences and such information charted in the resident’s medical record as outlined in our established policies and procedures. • Assist the Charge Nurse on investigation of alleged abuse or unusual occurrence as outlined within our established policies and procedures. • Start and monitor intravenous medication as ordered. • Assist the Charge Nurse and other nursing personnel in performing nursing procedures as necessary. • Assist the Charge Nurse in admitting, transferring and discharging residents. • Assist in correcting findings and deficiencies from the Department of Health Services, Consultants, and the Quality Assurance Team.• Assist in completing restraint assessments, pain assessments, oral assessments, psychotropic assessments, bowel and bladder assessments and I & O weekly evaluations. • Assure that all nurse’s notes are charted in an informative and descriptive manner that reflects the are provided as well as the resident’s response to the care. • Attend and participate in staff meetings and in-service classes. • Others as may become necessary and appropriate or as may be directed by the Director of Nursing Services or Administrator. SUPERVISION • Make resident rounds to ensure appropriate care is being rendered, identifying and making corrections as necessary. • Meet with nursing personnel to assist in identifying and correcting problem areas and/or the improvement of resident care. • Assist in making rounds with the attending physician as necessary. • Assist the Charge Nurse in revision and distribution of assignments in case of nursing shortage. • Assist in the supervision and direction of nursing personnel. • Participate in disciplinary action and evaluation of nursing personnel. CONSUMER SERVICE • Presents professional image to consumers through dress, behavior and speech. • Adheres to Company standards for resolving consumer concerns. • Ensures that all resident rights are protected. 
Job Description Under general supervision, guided by the policies and procedures of the Center for Reproductive Health and Ambulatory Services, and in collaboration with the physician staff, the Registered Nurse performs established nursing procedures using clinical knowledge to care for patients.The RN collaborates with the physician in providing excellent clinical care to the reproductive health patients.S/he consistently projects a professional and courteous demeanor.The RN demonstrates competence in the management of patient flow and daily peri-operative operations.The RN shall comply with the medical center house and telephone standards.S/he provides expert nursing care to the patients receiving assisted and non-assisted reproductive techniques.These duties include patient instruction and discharge teaching; charting and documenting in appropriate records; administering and monitoring medications; assisting with procedures; communicating all patient scheduling needs with practice staff; and coordinating patient care between the various practice teams.S/he will assess patient needs and in collaboration with the physicians’ plan interventions; evaluate patient response to treatments; admit, recover and discharge patients.The RN will work collaboratively with the HA staff, anesthesiologists, MDs, RNs and embryology team: and participate in the EMR/APEX process. The Registered Nurse will assess health needs and perform functions specific to unassisted and assisted reproductive techniques as well as andrology, gynecological treatment patients, and male Urology patients. S/he will assess patient needs, then plan, evaluate and participate in care for patients/couples. S/he will utilize specialized knowledge and skills for evaluation and teaching within this patient population and may act as a resource person within the organization and/or community. S/he will consistently project a professional and courteous demeanor. The Registered Nurse shall comply with the Medical Center house and telephone standards. Registered Nurse will: set-up for and assist physician with Oocyte Retrieval, Ultrasound Guided Embryo Transfer, Hysteroscopy, Manual Uterine Aspiration and Cyst Aspiration; set up and assist with male Urology procedures as needed including providing IV conscious sedation; check in patient, obtain vital signs, obtain IV access and complete preoperative nursing assessment. In addition, other duties include medication scheduling, prescription management; coordination of patient’s treatment cycle, education, and support throughout the continuum of care; and will provide accurate and safe postoperative care. Completes an ongoing pain assessment and management plan, as needed. Assesses discharge and teaching needs at the time of admission and throughout each episode of care. Planning Uses the nursing assessment and other appropriate resources to formulate a multi-disciplinary care plan. Revises the multi-disciplinary care plan according to the current condition and changing needs of the patient. In response to data indicating potential or actual risk to individual’s health, initiates action to correct, reduce or prevent the risk. Provides and documents a rationale for actions or decisions based on scientific principles, standards of nursing practice, previous experience, and/or clinical judgement. Department Description The Intensive Care Nursery (ICN), founded in 1964, is a regional tertiary center and has the proud distinction of being one of the first neonatal intensive care units in the world. In addition to caring for extremely premature infants, the ICN also cares for infants with congenital anomalies in need of evaluation and surgical interventions. The ICN opened the first Neuro Intensive care unit for newborns in 2008. Today, the ICN continues to enjoy a reputation for being at the forefront of innovation in the fields of neonatology and fetal treatment. Neonatologists, neurologists, radiologists and nurses work to improve neuro developmental outcomes in very premature infants and full-term babies who are at high risk of neurological injury or have clinical evidence of encephalopathy, seizures or stroke. The ICN provides state of the art care for infants with neurological, gastrointestinal, renal and skeletal, cardiac and respiratory diseases. We are one of a few centers in Northern California that provides ECMO and are recognized as an ECMO Center of Excellence. Staff nurses are the backbone of the ICN. They provide comprehensive, compassionate and highly specialized care to the most fragile of infants and their families. Job Description Under general supervision, guided by the policies and procedures of the Center for Reproductive Health (CRH) and UCSF Ambulatory Services, and in collaboration with the physician staff, the Registered Nurse (RN) role crosses care settings to promote health and healing for a specific patient population. The primary focus of the role is to improve access, timeliness to treatment and care coordination within the healthcare organization. The nurse performs established nursing procedures using clinical knowledge to care for patients. The RN collaborates with physicians and other members of the multidisciplinary team in providing excellent clinical care to all patients attending CRH for medical services and treatment. The nurse consistently projects a professional and courteous demeanor. The RN is a licensed registered professional nurse who is a clinical expert and actively promotes the delivery of evidence-based nursing interventions, demonstrates competence in the management of patient flow and daily clinic operations, and possesses expertise in the specialty nursing area and specific patient population of REI (Reproductive Endocrinology and Infertility). Job Description The clinical Nurse/RN is a professional caregiver who utilizes the nursing process for assessment, planning, implementation, and evaluation to provide safe, therapeutic care to patients, and also provides teaching of groups to patients and families for a designated time frame. Job Description The Adult Medical Transitional unit (30 beds) provides care to a diverse adult patient population with an emphasis on acute and chronic medicine patients. These patients require more frequent interventions (every two hours) and more nuanced nursing assessments. Patients transition from the adult critical care units, Post Anesthesia Care Unit, Emergency Department and acute care floors to this unit, depending on their condition and nursing care needs. Patient population includes but is not limited to end stage renal disease, DKA, new arrhythmias, respiratory compromise, sepsis screening and infectious diseases. Patients on 14 Moffitt commonly have central lines, wound care, nasogastric tubes, Q2 medications, electrolyte replacements and require maximum assistance for activities for daily living. All beds have telemetry and continuous pulse oximetry monitoring capability. Nurses on this fast-paced, high acuity unit are a highly motivated group of professionals providing outstanding care to the entire patient populations served. Teamwork among our nursing staff, as well as physician and nurse collaboration, is well tuned and allows workflow to occur optimally.
