Hampton, Virginia * We required Labor Pool to work 4 12-hour shifts a scheduled period with at least 2 of the shifts being on the weekend and we required 2 holidays a year. * Overview Works under the supervision of the Nurse Manager and the Director and is responsible for the assessment, planning, implementing, and evaluating the total nursing care of assigned patients. The RN utilizes communication, critical thinking, problem solving, and decision making to create a care plan for each patient based on physiological, psychosocial and cultural diversity. Responsible for supervising and delegating non-professional personnel working directly with him/her to ensure optimum patient care. Must demonstrate competence in dealing with age related needs of the specific patient populations served in nursing unit. Works under the direction of leadership and a multi-disciplinary team to provide a comprehensive range of services or clinical care associated with and in accordance with standards of excellence established by Riverside Health system and facility goals and strategic plans. What you will do Demonstrates ability to assess patient clinical and psychosocial needs by applying standards of practice as it relates to the specialty or patient population. Demonstrates nursing unit specific competencies and equipment training, see checklists. Partners with patients and their family to develop, implement and evaluate a plan of care, treatments and procedures that contribute to physiological and psychosocial outcomes of the patient. Incorporates evidence based standards of practice and collaborates with interdisciplinary team in care coordination. Facilitates safe patient transition to next level of care as part of plan of care. Demonstrates sense of urgency to facilitate patient throughput to ensure patient receives consistent, competent and ethical care. Focuses on quality outcomes of patient when addressing service needs to ensure evidence based quality care of patient. Demonstrates understanding of diverse patient care populations. Modifies education method to ensure patient/family understanding. Assumes responsibility to coordinate and deliver individualized patient education and discharge specific education to family/caregivers. Evaluates for comprehension and documents. Accurately completes all required clinical and quality documentation consistently per RHS standards and regulatory requirements. Delegates and prioritizes tasks appropriately to facilitate quality patient care and service excellence to meet the patient's care needs. Recognizes and responds to emergency situations. Evaluates resources and support needed and uses systems to ensure needs are met. The Day Center CNA ( Certified Nurse Assistant) works at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care. Follow the care plan and assist participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting. Document care provided in accordance with established policies, guidelines, and applicable regulations. Carry out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews. Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed. Monitor participants health status such as vital signs, weights, skin condition, appetite and changes in functional ability and report pertinent information to clinical supervisor in a timely manner. Collaborate with multidisciplinary teams in care planning for participants. Monitor appetite and look for changes in functional ability and skin condition. Monitor/assist participants in a courteous manner with meals and helps with feeding, as necessary. Report any variance, accident, or unusual occurrence to the appropriate personnel. Assist with loading and unloading of participants from vehicles. Assist the transportation department with participants’ take-home supplies. Serve as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments supporting participants while in transit and accompanying participants in and out of facilities. Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar when needed. Attend in-service and continuing education programs as scheduled and necessary.
