As the Hospice Nurse Practitioner, you will provide consultation in palliative care, symptom management and supportive care to meet the needs of the Hospice patient as requested by the referring physician. Must be able to travel throughout the agency's service area, which covers a 50-mile radius. Primary Responsibilities: Makes face-to-face encounter visits within prescribed time limits to qualifying Hospice patients and reports clinical findings to the Hospice Medical Director Consults with the Hospice Medical Director as needed, informs primary physician of recommended services, and collaborates with other physicians as needed Initiates and coordinates the plan of care. Documents problems, appropriate goals, interventions, and patient/family response to hospice care. Pulaski Health & Rehabilitation Center has an exciting opportunity for a Unit Manager, RN in our 102 bed skilled rehab center in Pulaski. The Unit Manager will oversee clinical operations of the unit and is responsible for staff supervision of nurses and CNAs, as well as interfacing with physicians, QI reporting and follow-up, and providing clinical expertise to staff as needed. Through a structured preceptorship on your home unit and monthly cohort sessions, you’ll develop clinical reasoning, decision-making, leadership and evidence-based practice skills while learning alongside other new graduate nurses. Experienced preceptors, nursing leaders, Staff Development Specialists and Advanced Practice Nurses will support you throughout the approximately one-year, paid residency program and beyond.
