This position requires a minimum commitment of 16 hours per month, including one weekend per month. Candidates must be able to provide care within a 20-mile radius of Doylestown Hospital. This is a rewarding opportunity to deliver compassionate, patient-centered end-of-life care in the comfort of patients' homes. If you do not have direct hospice experience; you must be able to train 2-3 times a week for 4-6 weeks. Summary: Our Home Hospice team is dedicated to providing compassionate, patient-centered end-of-life care that allows individuals to remain in the comfort and dignity of their own homes. We support patients whose curative treatments are no longer effective or who have chosen to focus on comfort and quality of life. Using a multidisciplinary approach, our hospice professionals develop personalized care plans that address each patient's physical, emotional, and spiritual needs. Team members may include registered nurses, certified home health aides, social workers, chaplains, and bereavement counselors who work together to provide comprehensive support for both patients and their families. Services include symptom and pain management, care coordination, emotional and spiritual support, caregiver education, medical equipment and medication management, and bereavement services. Assesses, implements, and evaluates care needed by assigned patients under the direction of the licensed physician. The RN is knowledgeable about principles, theories, current methods and techniques of nursing practice. Using the nursing process, the RN collaborates with the patient, patient's family member(s)/significant others, professional peers and other members of the health care team to individualize patient care. Directs nursing support staff in the provision of patient care according to the standards of nursing practice, administrative and departmental policies and procedures. Professional education, training or additional certifications may be required by specialty area.
