The Assisted Living Director Of Nursing – RN (AL-DON) is responsible for planning, organizing, directing, supervising, coordinating and evaluating the facility’s clinical services. The AL-DON ensures that staff are appropriately trained, supervised and follows facility procedures. The AL-DON is responsible for maintaining excellent quality in facility services and for assuring that services meet all applicable state requirements. Is available for on-call medical questions and/or emergencies via phone and provide appropriate nursing interventions. ESSENTIAL DUTIES AND RESPONSIBILITIES: · Assist the LALD with establishing the facility’s policies regarding the types of services we will provide and other policies regarding the scope of our services. · Work with the LALD in communicating the scope of services directly and through the UDALSA. · Responsible for development and implementation of written staffing plan to ensure adequate direct-care staffing is in place to meet resident needs 24 hours a day. · Periodically audit records and supervise staff to assure that staff are following procedures, are providing quality services and are maintaining resident records according to policy and state requirements. · Provides direct supervision of new RNs, LPNs & direct care staff within 30 days of date the staff person begins work and thereafter as required or as appropriate. · Assures clinical compliance with regulations and is available for state licensure surveys. · Complete LifeConnect Profiles and regulatory assessments for new residents or those with changes in cognitive, functional, or medical status. Conduct ongoing evaluations to monitor residents’ conditions and adjust care plans accordingly. · Develop and maintain individualized service plans that reflect each resident’s current care needs. Ensure all care plan updates are properly reviewed, signed, and documented by required parties. · Perform clinical assessments for potential residents, including completion of the LifeConnect Mini-Profile. · Collaborate with LALD and the AL Sales Specialist to determine and communicate appropriate service levels. · Document all relevant health information and communicate changes to families, physicians, and appropriate team members. · Administer medications that cannot be delegated to unlicensed staff; oversee medication setup and management. · Assist staff with daily responsibilities when appropriate. What You’ll Do The Psychiatric and Mental Health Nurse Practitioner (PMHP) serves on the Overnight Shift as a clinical provider within the Access & Assessment Department, practicing at the top of licensure and scope in accordance with Hospital Medical Staff Bylaws, Rules and Regulations of the Medical Staff, and clinical privileges granted by the Medical Executive Committee (MEC). Assists resident based on individual care plan with personal cares such as; bathing, oral hygiene, grooming, toileting, dressing, showers and responds to call lights Nursing Assistant may per community patient care model check and record vitals AM/PM/NOC cares; including peri care and catheter care May turn, lift or transfer residents with or without other persons/mechanical assistance Observes residents and reports status to appropriate staff in a clear and concise manner Chart or document in electronic medical record system Seeks to resolve resident and family concerns Receives and gives report to appropriate staff during various times in work shift Other duties such as; order and cleanliness in rooms and work areas, reports malfunctioning equipment to maintenance department, and answers phones in a timely manner Required: Active as Nursing Assistant, Registered (NAR) on the MDH Nursing Assistant Registry by start date – OR- Individual who is hired into Ebenezer’s Nursing Assistant Course or Ebenezer’s Pre-CNA job for Nursing Assistants; will be granted a 4-month post hire to become active on the Minnesota Nursing Assistant Registry.
