Purpose of Your Job Position The primary purpose of your position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by CNAs/GNAs and other nursing personnel. To monitor the performance of CNAs/GNAs, nursing and non-licensed personnel, provide education and counseling, perform disciplinary action as necessary, and complete performance evaluations. Contribute to the formulation, revision, and implementation of policies and procedures. On-Call Rotations for after hour services. Perform direct patient care duties as needed. Maintain compliance with professional standards and principles. Manage a team of Clinicians and follow up to ensure all necessary documentation is completed timely and follow up actions if indicated. Qualifications: Licensed Practical Nurse (LPN) with current licensure in New York Bachelor’s degree in nursing or related field preferred Coding Certified ICD 10 preferred Previous experience in home care or healthcare management preferred Strong leadership and communication skills Ability to prioritize and multitask effectively Proficient in Microsoft Office Excel, Microsoft Word and electronic medical records systems Verbal and written communication skills in English.
