Overview Provides high quality comprehensive nursing care to meet the needs of the patient/client and manages/coordinates the delivery of cost effective multidisciplinary health care services for a caseload of patients/clients consistent with VNS Health philosophy, policy, goals and objectives, and Standards of Nursing Practice. Works under moderate supervision. For Nurse-Family Partnership (NFP) only: Provides health education, social support and community linkages to meet the needs of pregnant and postpartum, first-time mothers and their infants. Also responsible for adhering to NFP national guidelines. • Assesses the physical, social and psychological needs and environmental conditions of the patient/client through professional knowledge, skills of observation, interviewing, and physical examination to develop a plan of care. • Understands, interprets, and implements reimbursement protocols; determines the billing source for patient/client care. Consults with management on complex/unique reimbursement cases. • Acts as the coordinator of care for the multidisciplinary team by providing and managing the care needs of patients/clients which includes making/confirming referrals to other disciplines/community agencies and ordering supplies/services and ensuring they are delivered as needed. • Documents content and process of all services provided in accordance with government regulations/requirements, and VNS Health policies, practices, and procedures, as well as NFP requirements as appropriate. Position Summary: We have an exciting opportunity to join our team as a Nurse Manager Community Medicine Program- FHC- (Healthcare for Homeless ). In this role, the successful candidate Provides clinical leadership and management of a designated care area(s) on a twenty-four-hour basis, consistent with the philosophy, goals, and mission of NYU Langone Health. (AONL-Leadership) Leads and supports developing of new insights to complex situations, innovative designs, and programs/processes (AONL-Leadership) Minimum Qualifications: To qualify you must have a Current Registered Professional Nurse licensure in New York State. Basic Life Support Certification through the American Heart Association Master's degree in nursing required. At least three years of pertinent clinical practice. Certification in nursing leadership required Ability to apply nursing process toward achievement of specific outcomes in an organized fashion to address patient/family satisfaction and quality indicators. Excellent communication (both oral and written), interpersonal, organization, critical thinking, problem solving, and leadership skills.. Required Licenses: Registered Nurse License-NYS, Basic Life Support Cert Preferred Qualifications: Preferred 1 - 2 years Leadership or supervisory experience. Experience with the underserved population. Nursing Leadership certification required Qualified candidates must be able to effectively communicate with all levels of the organization. NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $126,000.00 - $154,791.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
