Full job description The after hours nurse supports our patients and families needs after hours. After hours is qualified as 5 p.m. to 8 a.m. and weekends. Anchor Health is seeking a Registered Nurse and Licensed Vocational Nurse to serve in the San Mateo County . The Registered Nurse will plan, organize and direct hospice care. They will also build from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Position Responsibilities: - Coordinating the care of the patients assigned to you. - Making regular visits to patients' homes to assess patients' physical, emotional, spiritual, and social needs. - Working collaboratively with the hospice physician and your supporting LVNs to carry out appropriate interventions. - Helping the agency to continuously increase the quality of care.
