The Advanced Care Practice Manager is responsible for the overall management of the clinical program. This position oversees and manages the daily clinical and nursing practice for programs within the scope of the division, creates treatment plans, provides patient assessment, medical triage, and care coordination within the scope of APP licensure. This is primarily an administrative position, but direct patient care is required within the scope of the position. Ensures the clinical team is adhering to all regulations and policies while conducting patient care in the home.
