Collaborates with other members of the interdisciplinary team as needed to ensure residents’ needs are holistically met. Informs appropriate departments of concerns or complaints and files grievances as necessary. Reports any incidents or unusual occurrences to the supervisor, unit manager, assistant director or nursing or director of nursing and participates in the investigative process as needed. Participates in end of life care as required. Ensures that there is adequate stock of medications, supplies, equipment and notifies appropriate personnel of needs. Initiates, reviews and updates care plans as required. Promotes teamwork, mutual respect, and effective communication. Participates in the survey process when inspection surveys arise. Reports any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to administrator. Protects residents from abuse, and cooperates with all investigations. Reports any occupational exposures to blood, body fluids, infectious materials, and/or hazardous chemicals in accordance with facility policy. Participates in all life safety and emergency drills and trainings. Fulfills responsibilities as assigned during implementation or activation of the facility’s emergency plan.

Ensures the delivery of high-quality service by supervising activities of assigned HHA staff. Maintains compliance of HHAs with corporate policies, state regulations, and contractual requirements. Initiates appropriate corrective action and documentation by coaching, counseling, and terminating HHAs as required, in a timely effective manner. Encourages and develops on-going communication with HHA staff and management across VNS Health; identifies and resolves issues which effect service delivery. Maintains or grows caseload depending on resource availability. Develops and implements procedures to maximize operational efficiency and effectiveness, as necessary. Performs all duties inherent in a supervisory role. Ensures effective staff training, interviews candidates for employment, evaluates staff performance and recommends hiring, promotions, salary actions and terminations, as appropriate. Participates in special projects and performs other duties as assigned.
