Overview Missouri Baptist Sullivan Hospital is a designated critical access hospital committed to providing quality health care service to residents in Crawford, Franklin and Washington counties. The hospital has enhanced and expanded the health care services available to the greater Sullivan community with its state-of-the-art emergency department, modern childbirth center, critical care unit, multiroom surgical suite, expanded cardiac rehab services, and a cancer center. A 22,500 square-foot medical office building provides offices for primary care physicians as well as space for a variety of visiting medical and surgical specialists from the St. Louis area. Missouri Baptist Sullivan Hospital operates four rural health clinics providing quality health care to surrounding communities. In addition, the hospital operates a comprehensive Therapy and Rehabilitation department and an award-winning Wound Care Center with two hyperbaric chambers. The Sullivan medical clinic is a well-established, multi-discipline physician group providing family, internal and OB care. Preferred Qualifications Role Purpose The Nurse Practitioner (Primary Care) utilizes a high degree of knowledge, skill, and competence to deliver patient care in a primary care clinic setting. This position assesses, diagnoses, and initiates a patient care treatment plan within the scope of a formal collaborative practice agreement with a primary care physician. In this role, patient care is performed with considerable independence, guided by physician-approved protocols, and evaluated through chart review, consultations, and the observation of results under the general medical direction of an Internal Medicine/Family Medicine physician. Responsibilities Participates in professional development Manages the health care of patient populations with compassion through advanced health assessment, diagnosis, intervention, and evaluation in a collaborative practice agreement with a physician. Prescriptive responsibilities are reviewed by collaborative physician. Coordinates the care of patient populations, in a collaborative practice agreement with a physician, to ensure promotion of health and wellness through education of patients and families, coordination of services and placing referrals. Assesses development needs of staff and develops and delivers appropriate educational programs to meet staff needs. Develops treatment plans, treats patients with acute problems including illnesses and minor injuries following a predetermined protocol in a collaborative practice agreement with a physician. RESPONSIBILITIES I. JOB SUMMARY/RESPONSIBILITIES: • An Advanced Practice Provider (APP) is a medical provider who treats, diagnoses, prescribes, and/or performs or assists in surgical procedures. APPs are licensed healthcare professionals, credentialed and privileged through the Medical Staff Office. APPs work collaboratively with physicians and the care team in all areas of the hospital and clinics to create a seamless environment of care. Direct care services may include, but are not limited to, obtaining a patient history, providing a complete or focused physical examination, ordering, conducting and interpreting diagnostic and laboratory tests, performing procedures, patient and family education, and managing social, economic and ethical issues within the scope of a health care provider. Indirect patient care services may include, but are limited to setting standards for nursing practice and facilitating the professional practice of nursing care at the bedside. • If licensed as Physician Assistant (PA) (State of Hawaii Administrative Rules Chapter 85, Subchapter 6): o A PA is an individual who has been certified by the board to practice medicine with physician supervision. A PA may perform those duties and responsibilities delegated by the supervising physician. o Under the supervision of a Physician, the PA functions as an expert practitioner. The PA provides health care that includes services to individuals and families, which includes health promotion, disease prevention, diagnosis and management of acute and chronic disease. Direct care services may include, but is not limited to, obtaining patient histories, providing a complete or focused physical examination, ordering, conducting, and interpreting diagnostic and laboratory tests, various diagnostic and/or therapeutic procedures; and performing procedures, etc., within scope of practice and licensure. The PA also provides patient and family education, and management of social, economic, and ethical issues within the scope of the health care provider. II. TYPICAL PHYSICAL DEMANDS: • Finger dexterity, seeing, hearing, speaking. • Continuous: sitting, static gripping of an object for prolonged periods. • Frequent: walking. • Occasional: standing, stooping/bending, climbing stairs, walking on uneven ground, lifting and carrying usual weight of 1 pound up to 5 pounds, reaching above, at and below shoulder level, frequent gripping of an object. III. TYPICAL WORKING CONDITIONS: • Not substantially subjected to adverse environmental conditions. • May be exposed to communicable disease and body fluids, and chemicals. • May be required to deal with hostile patients and/or families. IV. MINIMUM QUALIFICATIONS: A. EDUCATION/CERTIFICATION AND LICENSURE: • Current Hawaii State license as an APRN or Physician Assistant in good standing • If licensed as an APRN, specialty board certification aligned with area of assignment is required. • If licensed as an APRN, successful completion of a Registered Nurse First Assistant (RNFA) Program is required. • Depending on area of assignment, prescriptive authority for non-controlled and controlled substances may be required. • Depending on area of assignment, additional certification(s) may be required. • Credentialing and privileging required if providing medical level of care. • Current ACLS certification. • Current BLS certification. The Knight Cancer Institute Center for Hematologic Malignancies (CHM) outpatient clinic provides care for multiple different types of hematologic malignancies including Lymphomas, Leukemias and Multiple Myeloma. We are seeking an Advanced Practice Provider who will provide clinical management of adult ambulatory patients with multiple myeloma to join our dedicated Myeloma Program. Function/Duties of Position Position Summary OHSU classification: Clinical Associate APP , represented by the Oregon Nurses Association & Advance Practice United, subject to the active collective bargaining agreement. This Clinical Associate-APP provides care in conjunction with a team comprised of physicians, APPs, RN coordinators, and other allied health professionals. APPs may practice both independently or in a shared-care model with physicians, as determined by the needs of the team. Additionally, this APP will perform procedures and may intermittently provide oversight to the infusion unit. The ideal candidate will thrive in a fast-paced, team-oriented environment and demonstrate clinical knowledge and oncologic experience to support a complex patient population. The Clinical Associate-APP delivers care aligned with a defined scope of practice in Outpatient Adult Medical Oncology , with work expectations operationalized through departmental workflows, productivity standards, and performance expectations consistent with the collective bargaining agreement and institutional policies. This role is structured to meet contract-defined work expectations, inclusive of daytime coverage, aligned with the clinical coverage model and patient care needs of the service.
