Collaborates with the physician to identify needs, to ensure pain and symptom control. Understands the CMS guidelines for Face to face visits. Maintains current knowledge of the latest research and trends in hospice care and pain/symptom management. Works with the IDT in a team approach, participates in performance improvement programs. Demonstrates knowledge in communications and counseling patient/family in dealing with end-of-life issues. About the Role We are seeking a skilled and compassionate Nurse Practitioner to perform required Face‑to‑Face (F2F) encounters for hospice patients approaching benefit period recertification. Key Responsibilities Conduct Face‑to‑Face encounters for patients entering their 3rd benefit period and beyond. Assess and document terminal prognosis, disease progression, functional decline, and symptom burden. Collaborate with hospice physicians, RNs, social workers, and the IDG to support accurate recertification decisions. Provide clinical insight and education to staff regarding disease trajectories, eligibility criteria, and hospice best practices. Maintain timely, compliant documentation aligned with Medicare and state regulatory standards. Communicate findings effectively to physicians and clinical leadership. Travel within assigned New Mexico service area to visit patients in homes, assisted living facilities, SNFs, or other care settings. Qualifications Current and unrestricted Nurse Practitioner license in New Mexico. National board certification as NP (FNP, AGNP, or similar). Minimum 1-year clinical experience; hospice, palliative care, or geriatric experience strongly preferred. Solid understanding of hospice eligibility criteria and disease trajectories.
