Position Summary Vitae Health is seeking a dedicated Nurse Practitioner to provide high-quality, patient-centered medical care to residents in a skilled nursing facility. The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve patient outcomes and quality of life. Description Northwestern Medicine has an exciting outpatient opportunity for a Nurse Practitioner or Physician Assistant to lead the newly established Lung Health Center within CTI. The Lung Health Center is designed to improve lung cancer screening rates through comprehensive respiratory health assessments, patient triage, care coordination, and longitudinal follow-up. This role will support patients of all backgrounds and clinical complexities while advancing early detection and preventative lung health initiatives. The APP will be responsible for maintaining an outpatient clinic, both virtual and in person, developing and implementing individualized plans of care, and coordinating treatment with interdisciplinary team members and consulting providers. This role will also work closely with research teams as part of a broader lung cancer screening and early detection initiative.
