Summary Under the supervision and/or direction of a Nurse Manager, Charge Nurse or Staff Nurse, the NAR performs various duties assisting nursing or clinical staff in the treatment and care of patients in accordance with the established methods and procedures of the Senior Care Unit and the host hospital. Patient Care - Under direction of the Charge Nurse, the NAR will perform the following patient care activities: Assist the assigned patients in activities of daily living. Routine Duties include, but are not limited to: Taking care of a patient's personal hygiene, including bed baths, shaving, nail care etc* Toileting patients and emptying catheter bags* Monitoring vital signs including temperature, pulse, respiration and blood pressure* Setting up of meal trays, documenting food/fluid intake and feeding patients* Serving nutritional supplements* Making beds and keeping patient's space clean and tidy* Bringing the patients to therapy area* Turning bed ridden patients to prevent bedsores* Encouraging and assisting with ambulation* Observing patients* Assist patients in maintenance of their environment, as required.* Participate in planning and implementing daily activities for patients. Monitor the patient attendance in Unit Activities for patients.* Interact with patients on a one-on-one basis in a therapeutic manner.* Report relevant patient information to the RN for evaluation, intervention and documentation.* Monitor the patient during the use of potentially hazardous equipment.* Observes patients and communicates changes and/or needs to the RN.* Documents patient cares in the Electronic Medical Record. Overview NaphCare is hiring Licensed Practical Nurses to join our team of medical professionals in Grand Junction, Colorado, where we manage all aspects of healthcare for the individuals who are currently incarcerated in the Mesa County Jail. We currently have PRN positions available! Pay rates range from $28.00-$30.45 per hour. Pay rates range per hour based on employment status, site needs, licensure, and experience. As a Correctional Nurse , you will work alongside correctional officers and other medical professionals to provide care to the individuals in residence. Working within a controlled environment, where security is top priority and the days are never dull, you will have various duties to keep you engaged including performing physical assessments, administering medications, managing chronic illnesses, and responding to emergencies. This is a unique position where you can make a difference to this diverse population of patients with complex medical needs. *New grads are encouraged to apply - we'll train you in the exciting field of correctional healthcare! * Responsibilities As an LPN , you will play a critical role in our continued mission to provide quality healthcare that protects the rights, dignity, and health of our unique patient population. Duties and responsibilities may include and not be limited to: Performing patient assessments, interviewing patients about current health problems, medication history Taking vital signs including blood pressure, temperature, and weight Providing wound care, including cleaning and bandaging injured areas Administering medication, including injections of medications and monitoring reaction to meds Maintaining medical records, including entering information into our electronic health record, TechCare Observing patient health and mental health status Providing emergency first aid Providing patient education and discharge/release planning Qualifications Experience / Requirements / Qualifications Graduate from a Licensed Practical Nursing program High school diploma or equivalent (GED) Actively state-certified as a Licensed Vocational Nurse or Licensed Practical Nurse Must possess an active BLS CPR Certification (American Heart Association preferred) Able to maintain confidentiality of all proprietary and/or confidential information Display integrity, professionalism and be able to adhere to a Code of Conduct and comply with all facility(s) correctional healthcare policies, procedures, and legal requirements Must pass the facility’s criminal background check and drug screening Physical Capabilities: Must have the ability to stand for long periods, lift and move patients (up to 50 pounds), bend, squat, reach, push, pull, walk significant distances, and perform physically demanding tasks like CPR, all while maintaining good balance and endurance throughout a shift. Overview SSM Health Neuro Transitional Center A Joint Venture between Select Medical and SSM Health Position: Assistant Residential Manager (RN) Location: O'Fallon, MO Schedule: Full-Time Monday-Friday, 8 hour days Sign-On Bonus: $15,000 - NO COMMITMENT Responsibilities The Assistant Residential Administrator must be an RN. Will be responsible for oversight of day to day medical coordination, assist with nurse direction of LPN and direct patient care to maintain patient medical stability within the Neuro Transitional Center in accordance with Residential Care Facility guidelines. Will also participate in nurse on call duties. Transitional rehabilitation is dedicated for individuals with traumatic brain injury (TBI), spinal cord injury* (SCI - C5 and below; not on vent), stroke or other neurological diseases, and who are also at high risk for unsafe discharge. Our Neuro Transitional Center provides a unique environment designed to feel like a home, yet gives patients with physical, behavioral and cognitive impairments access to comprehensive therapy in multiple settings. Specialized therapists help transitional patients restore independent living skills — with the goal of safely functioning in their home and community. Our new Neuro Transitional Center is another way we're changing rehabilitation for the better.
