Facilitates care via telehealth to assist with patient needs and concerns. Refers cases identified as risk management issues, peer review issues, or quality issues to appropriate personnel. Overview To support the Mission of Helio Health, Inc., the CTI Licensed Practical Nurse will provide comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a “critical time” transitional period in their lives. CTI Licensed Practical Nurse: CTI is designed specifically to enhance the continuity of care during transition from hospital to community care. The schedule is Monday-Friday, 8:00am-4:30pm, with one late shift per week (12pm-8:30pm), and some evening on call responsibilities.
