The Triage RN reports to Associate Clinical Director. He/she is responsible for triaging patient and caregiver telephone calls, assisting in the solution, palliation of symptoms and provision of education. Documents discussions in detail. • Assists with pharmacy issues (ordering medication refills, securing scripts for controlled substances, problem solves breakdown in systems communications). • Orders supplies and durable medical equipment as appropriate and documents same in the EMR • Coordinates equipment pick up with vendor representative as necessary. • Trends calls prospectively to determine patterns by team/provider and identifies opportunities for improvement. • Deploys staff as needed to address patient and family assessed needs. Demonstrates knowledge in usage, indication, adverse side effects, contraindications and negative interactions of most commonly used steroids, NSAIDs, benzodiazepines, antidepressants, anticonvulsants, narcotics, antifungals, and diuretics. Documents to agency standards, timely completion, timely signatures, and documents care using A-I-R protocol to support hospice stay in program and level of care. Caregiver Job Duties Our caregivers support their clients in activities of daily living such as (but not limited to): Household chores (cleaning, laundry, dishes, etc.) Cooking and/or serving meals Helping clients bathe, dress, and groom Providing companionship through daily activities and hobbies Driving clients (as needed) to the store or appointments Monitoring and reporting on their condition Additional Information Our hiring process is fast and simple! Once you have completed onboarding, orientation, and training, you can start working with clients! You will be able to complete the training while working and will have 120 days to complete it. Requirements 18 years+ Ability to lift a minimum of 10lbs and up to 50lb rarely.
