JOB DESCRIPTION " Position Summary The Transition of Care Nurse provides consultative and coordinating functions in the transitioning of patients from the acute care facility to home or alternative care sites. This position works in conjunction with the Nursing and Medical/Case Management departments of the hospital and at the direction of the management of AtlantiCare LIFE Connection. The Transition of Care Nurse also is responsible for the coordination of care of all AtlantiCare LIFE Connection participants currently residing in an acute care setting. This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork and cooperation. Qualifications EDUCATION: Graduate of an accredited school of Nursing required. Bachelor Degree in Nursing preferred. LICENSE: Current Licensure as a Registered Nurse in the State of New Jersey.
