Minimum Education: Has completed the basic professional curricula of a school of nursing as approved and verified by a state board of nursing, and holds or is entitled to hold a diploma or degree therefrom. Licensure, Registration and/or Certification: Valid multi-state or State of Oklahoma Registered Nurse License. A valid driver's license and a Motor Vehicle Report that meets SFHS guidelines are required. Work Experience: Minimum 1 year of related experience, preferred. Good clinical, interpersonal and communication skills. Essential Functions and Responsibilities: Performs assessment to determine the patient's eligibility and needs for home health services. Develops & revises a patient-centered plan of care based upon the patient's strengths, stated goals and care preferences. Delivers nursing care with consideration for the patient's cultural, psychological, functional, cognitive & social determinants. Includes the patient & family in the care planning process. Organizes & prioritizes case management responsibilities including complex tasks. Demonstrates professional judgment in response to changes in patient's status and agency's constraints. Completes assigned work within allotted time within the productivity standard. Identifies ongoing patient needs, initiates appropriate and timely referrals & coordinates care with the health care team. Completes daily laptop communication and downloads updates timely. Reviews all medications the patient is using in order to identify any potential adverse effects, drug reactions, including ineffective drug therapy, significant side effects, significant drug interactions, duplicate drug therapy, and non-compliance with drug therapy. Communicates any medication issues with the health care team. Keeps an updated medication list in the home. Demonstrates knowledge of resources & initiates measures to establish an effective medication system within the patient's home. Documents patient's care on the appropriate forms in EMR. Accurately records data accurately in the EMR according to the plan of care. Utilizes correct medical terminology and abbreviations. Documents coordination of care and progress toward goals and outcomes. Develops and revises the Home Health Aide plan based upon the ability for the agency to safely meet the patient's needs with their ADL's and IADL's. Conducts & documents the Home Health Aide supervisory visit at least every 14 days. Performs nursing skills in accordance with assessed needs and physician orders. Anticipates and delivers patient care according to agency processes and policies. Instructs patient and families in emergency, safety, and home management of their disease or illness. Demonstrates knowledge and proficiency in clinical skills required in the home health field. Ensures that each patient, and/or caregiver, receive ongoing education and training regarding the care and services in the plan of care, to ensure a timely discharge plan. Communicates any revisions of the plans for patient's discharge to the patient, representative, caregiver, physicians and health care team who will be responsible for providing care and services to the patient after discharge from the agency. Supports agency hospital goals through SHP analysis, PDGM case conferencing, one on one case conferencing, QA, educational standards and QAPI. Makes QA/SHP changes timely and accurately. Modifies behaviors or performance to comply with standards and meet thresholds. Assists with orientation process as directed. Supports an interdisciplinary team approach. Complies with the Home Health Infection control plan. Follows the infection control policies and practices of the agency. Decision Making: Independent judgment in making decisions involving non-routine problems under general supervision. Working Relationship: Coordinates activities of others (does not supervise). Works directly with patients and/or customers. Works with internal/external customers. Works with other healthcare professionals and staff. Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients promptly as per Agency policy. Coordinates services. Informs personnel of changes in the condition and needs of the patient. Counsels the patient and family/significant others in meeting nursing and related needs. Participates in and presents in-service programs. Understands and adheres to established Agency policies and procedures. Processes orders and notifies physicians of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries. Determines the amount and type of nursing needed by each patient. Refers to Physical Therapists, Speech Language Pathologists, Occupational therapists, and Medical Social Workers those patients requiring their specialized skills. Supervises and teaches other nursing personnel. Conducts patient care conferences on patients assigned to his/her care. Participates in peer review Quality Assurance and Performance Improvement as assigned. Gives total patient care as needed. Takes on-call duty nights, weekends, and holidays, as assigned. Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharge, and significant changes in condition by agency defined time frames. Overview Coordinates and provides high quality nursing care to Hospice patients and families consistent with Hospice Care philosophy, policy, goals and objectives, and Standards of Nursing Practice. Works under moderate supervision. SCHEDULE: Admissions RNs can work either 7.25, 4.50, or 21.75 hours per week (1-3 shifts). Number of hours worked, as well as specific days worked are flexible based on the employee's schedule week to week Shifts are scheduled from 1!:00am-7:00pm, Monday-Sunday • Assesses the physical, social, psychological, living conditions and pastoral care needs of the patient/family through professional knowledge and skills of observation, interviewing and physical examination on an ongoing basis. • Establishes and revises, as necessary, the nursing component of the patient/family plan of care based on goals mutually acceptable to the patient/family and significant others and makes referrals to other services, as necessary. • Coordinates implements and evaluates the effectiveness and outcome of the care plan by ongoing assessment, planning, coordination, counseling, teaching and supervision. • Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient. • Transports and utilizes required medical equipment and supplies using VNS Health designated/supplied carrying case weighing approximately 25 – 30 lbs. to and from patient homes/care facilities, VNS Health offices and other locations. • Provides nursing care in accordance with Agency policies, practices, procedures and Standards of Nursing Practice, which may require standing, stooping, sitting, crouching, bending and stretching to deliver patient care. • Assists patients with activities of daily living, which may require positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals. • Visits clients every two weeks to assess the clients’ condition and update the plan of treatment. • Monitors the effect of the plan of cares on the patient/family and informs physician and other personnel of changes in the patient's needs. • Manages a caseload of patients. • Provides nursing care services and initiates appropriate preventive and rehabilitative nursing procedures. • Participates in interdisciplinary team meetings. • Appraises care provided and revises the goals and approaches to patient/family care in coordination with the interdisciplinary team. • In coordination with the interdisciplinary team, assesses the family's needs during the bereavement period and provides ongoing counseling with referral to community resources, as appropriate. • Initiates and maintains written (including clinical and progress notes) and verbal communications to assure optimal patient care. • Understands and interprets Hospice Care reimbursement policy and determines the billing source for patient care. • Maintains productivity sufficient to meet Hospice goals. • Participates in the orientation of new staff. • Orients, instructs and supervises License Practical Nurses, paraprofessional staff and volunteers. • Consults with and educates the client and family regarding the disease process, self-care techniques, and prevention strategies. • Performs established work procedures in relation to Region/Program workflow. • Understands and interprets the Hospice philosophy and responsibility to other health professionals, laymen and community. • Assumes responsibility for professional growth. • Evaluates conformance of the inpatient facility in carrying out the interdisciplinary team's plan of care for select patients, when assigned. • Participates in special projects and performs other duties as assigned.
