Job Summary The Registered Nurse (RN) works as an integral member of the inter-professional collaborative practice team administering community-based strategies aimed at improving the health of the community through education, health screening and coordinating evidence-based programming. Responsible for utilizing the nursing process to provide nursing care to assigned area including shelters and non-structured community based settings across the continuum of care. Assists individuals in accessing specialized programs through the use of screenings and referrals. Considers evidence-based outcomes when selecting interventions. Depending on location and home health needs, may perform as a respite care nurse. Maintains relationships with the local and regional community, including churches and other service organizations, to promote continued support and capacity building. May assist in organizing and performing community flu shot clinics based on needs of the population. Identifies, strengthens and links with the community's tangible resources, including schools, businesses, local service groups, and intangible resources such as community spirit. Recruits, trains and retains community volunteers. Demonstrate critical thinking skills to analyze data and appropriately respond to changing or difficult situations. The nursing process is utilized to assess, diagnose, establish a patient centered plan with defined outcome criteria; this plan includes interventions and means of evaluating the human responses to actual or potential health problems. Assumes the responsibility for the outcome of nursing care during a specified work period when they are present in the community setting. This practice includes dependent, interdependent and independent functions. Practices within the full scope of nursing expertise with knowledge of and utilizing appropriate age related nursing standards. Functions within the scope and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct. Qualifications Bachelor’s degree in nursing preferred. Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications. PRINCIPAL DUTIES AND RESPONSIBILITIES: All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations. · Performs all essential functions under the direction of the Supervisor with guidance from the Educator, Preceptor or in collaboration with another Registered Nurse. · Performs ongoing, systematic collection and analysis of dialysis data for assigned patients and documents in the patient medical record, makes adjustments or modifications to treatment plan as indicated and notifies Supervisor or physician as needed. · Assesses, collaborates and documents patient/family’s basic learning needs to provide initial and ongoing education to patients and family. · Directs and provides, in collaboration with the patient, home care partner, direct and ancillary patient care staff, all aspects of the provision of safe and effective delivery of dialysis therapy to assigned patients. · Administers medications as prescribed or in accordance with approved algorithm(s), and documents appropriate medical justification and effectiveness. · Initiates or assists with emergency response measures. · Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned. · Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections. · Identifies expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team. · Ensures patient awareness related to transplant and treatment modality options. · Assists in the identification, evaluation, selection and education of Home Dialysis candidates and Home Partners. · Performs assessment and identifies barriers of the Home Dialysis candidate’s home environment and partner / family readiness and ability to perform dialysis treatments in the home. · Trains Home Dialysis patients and / or Home Partners on the safe, effective operation and maintenance of all Home Dialysis equipment and treatment supplies through an organized and formal Home Dialysis Training Program. · Required to complete CAP requirements to advance. · Performs all other duties as assigned by Supervisor. EDUCATION and LICENSURE: · Graduate of an accredited School of Nursing. · Current appropriate state licensure. · Current or successful completion of CPR BLS Certification · Must meet the practice requirements in the state in which he or she is employed.
