Position Purpose This position is responsible for the direct care of clients and for ensuring quality and safe delivery of home health care services, and for coordinating home health services within the team and at discharge from home health to ensure smooth transitions of care. The position provides services to adult and geriatric clients. Nature and Scope The Home Health RN is responsible to provide short-term, acute care home health services to homebound clients who may include the terminally ill and their families, complying with the agency policies and procedures. The incumbent must be highly motivated and demonstrate critical thinking skills. The position requires independent judgment and assessment skills, driving and on-call rotations are components of this position. The incumbent must demonstrate the knowledge and skills necessary to provide care based on physical, motor/sensor, psychosocial, and safety appropriate to the age of the clients served. Additionally, is responsible for knowing and understanding the requirements for health insurance including paperwork, timelines and Medicare rules, for coordinating with the care team, patient/caregivers, and providers, to ensure the team is working toward realistic plan of care goals.
