Sleep Nurse Navigator

The Sleep Nurse Navigator serves as the primary nursing and care coordination resource for Sleep Surgery patients, providing in-clinic support to the Sleep Surgery physician(Mondays all day, Tuesdays all day and Wednesday in the Morning).Responsibilities include preparing patients for clinic visits and procedures by reviewing medical records, ensuring all required testing and appointments are completed, and coordinating care throughout the patient journey. The Navigator communicates with patients via phone and MyChart to schedule appointments, obtain necessary clinical information, coordinate outpatient procedures, and facilitate sleep studies and follow-up visits. Additional duties include managing prior authorizations, reviewing adult and pediatric sleep study requisitions, determining appropriate sleep study types and advanced testing requirements, pending orders for provider signature, and identifying high-risk patients for provider review. The role also serves as the primary point of contact for Pediatric ENT coordination, Restless Legs Syndrome (RLS) iron infusion therapy planning in collaboration with pharmacy and infusion teams, and clinical trial patients requiring inpatient sleep studies or home sleep testing.
