Position Summary A Community Health Nurse is responsible for coordinating and providing personalized skilled nursing care to patients in their homes. This role involves assessing patients’ health needs, developing and updating individualized care plans, providing skilled clinical services such as wound care, drain management, catheter changes, medication management, and other nursing treatments. In addition, conducting case conferences with a multidisciplinary team that may include therapists, home health aides, dietitians, social workers, nurses, physicians, and other healthcare professionals. The Community Health Nurse monitors patient progress, coordinates services, educates patients and families about medications, treatments, and home safety, connects individuals with community resources, maintains accurate documentation, and ensures that care is delivered safely and effectively. Strong clinical knowledge, communication, organization, and care-coordination skills are essential for promoting patient wellness, independence, and quality of life.
