Develop, monitor, and update individualized health goals. Ensure infection control standards and universal precautions are followed. Maintain necessary medical and first aid supplies. Provide on-call phone consultation as needed. Partner with case management teams to coordinate medical and behavioral health services. Provide health education to individuals, families, and direct support professionals. Coordinate hospital discharges and community reintegration services. Arrange follow-up care with primary care providers, specialists, and community resources. Serve as a clinical liaison between providers, families, and program staff. Participate in interdisciplinary treatment planning and case conferences. Train and mentor Direct Support Professionals (DSPs) on health-related procedures and emergency response. Maintain accurate and timely clinical documentation. Qualifications Current unrestricted New York State Registered Nurse (RN) license. Minimum of two years of nursing experience. Experience working with individuals with IDD, behavioral health, crisis care, or community health is preferred. Knowledge of OPWDD, OMH, and DOH regulations is preferred. Strong assessment, crisis intervention, care coordination, communication, and documentation skills. Ability to work independently and collaboratively within a multidisciplinary team.
