POSITION DESCRIPTION : The incumbent is responsible for giving ordered treatments, administer prescribed medications, give direct ADL care and complete requirements for diagnostic procedures as requested by the attending physician on an assigned shift per established policies and procedures. On assigned residents and within the assigned time frame will: Complete, sign and date the MDS, RAP modules and additional assessments. Will review and update care plans; notify chairperson if changes need to be made per established policies and procedures; and attend Interdisciplinary Care Plans when requested. Will always maintain professional standards of nursing care. Follows all local, state, and federal rules and regulations, as well as the policies and procedures of the facility and nursing department. Attend mandatory and non-mandatory in-services, meetings, Care Plans and MDS as required. Will ensure economic utilization of supplies and proper charging of items to residents’ accounts. Join us as a Registered Nurse (RN) -Care Manager position at Physicians Regional Medical Center - Collier campus Unit: Care Management Shift: 8AM-4PM or 8:30AM-5PM- With rotating weekends Student Loan Contribution: Up to $20k Other incentives include: Medical, Vision, Dental, 401k match & more available for Full and Part-Time roles Job Summary The Care Manager - RN is responsible for coordinating and overseeing discharge planning, transitions of care, and case management activities to ensure optimal patient outcomes. This role involves collaborating with interdisciplinary teams, reviewing medical records for appropriateness and medical necessity, and maintaining compliance with federal, state, and accreditation standards. Essential Functions Conducts daily reviews of medical records to assess the appropriateness of admission, continued hospital stay, and utilization of diagnostic services. Collaborates with interdisciplinary teams (IDT) to ensure effective communication and coordination of patient care, including identifying avoidable days and resolving care transition issues. Develops and implements discharge plans, coordinating post-hospital placement and social services to meet patient needs. Refers cases to physicians or managers when patients do not meet established criteria, ensuring timely and appropriate interventions. Serves as a liaison with community agencies, maintaining relationships and facilitating seamless transitions for discharged patients. Facilitates interdisciplinary meetings to address patient care needs, resolve challenges, and support collaborative care planning. Maintains accurate and timely documentation of case management activities, including records of referrals, patient interactions, and compliance with reporting requirements. Identifies and appropriately refers cases to Child/Adult Protective Services, ensuring compliance with legal and ethical standards. Provides professional assistance to patients, families, and physicians regarding discharge planning and post-hospital care options. Performs other duties as assigned. Maintains regular and reliable attendance. Complies with all policies and standards. Qualifications Bachelor's Degree in Nursing preferred 2-4 years of clinical nursing experience in a hospital, home health, or nursing home setting required 2-4 years of care management experience preferred Knowledge, Skills and Abilities Strong understanding of case management principles, discharge planning, and transitions of care. Knowledge of federal, state, and Joint Commission standards related to case management. Excellent communication and interpersonal skills to collaborate effectively with patients, families, and interdisciplinary teams. Ability to assess complex situations, identify solutions, and implement care plans efficiently. Proficiency in electronic medical records (EMR) and documentation systems. Strong organizational and time management skills to prioritize tasks in a dynamic environment. Licenses and Certifications RN - Registered Nurse - State Licensure and/or Compact State Licensure required BCLS - Basic Life Support preferred State Specific Requirements Alabama: Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification preferred. New Mexico: Advanced Cardiovascular Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certifications preferred.

CORE JOB SUMMARY The Registered Nurse - Specialty delivers patient-family centered care in a culturally competent manner utilizing evidence-based standards of quality, safety, and service while ensuring population-specific patient care. The incumbent oversees nurses and allied health staff in their daily practice. CORE JOB FUNCTIONS 1. Assesses assigned patients and evaluates plans to include documentation of nursing care, and reports symptoms and changes in patients’ condition and vital signs. 2. Modifies patient treatment plans as indicated by patients’ responses, conditions and physician orders. 3. Reviews, evaluates, and reports diagnostic tests to assess patient’s condition. 4. Consults with physicians and other healthcare professionals related to assigned patients to assess, plan, implement and evaluate patient care plans. 5. Prepares patients for, and assists with examinations, procedures, and treatments. 6. Considers patient age and culture during patient treatments and provides any needed information regarding treatment plan. 7. Nurtures a compassionate environment by providing psychological support. 8. Performs appropriate patient tests and safely administers medications within the scope of practice. 9. Administers and maintains accurate records related to medications and treatments as per regulatory bodies, policies, procedures and physician orders. 10. Communicates plan of care in a timely manner to patient and family, as well as the appropriate team members, ensuring compliance with all regulatory guidelines (i.e., HIPAA). 11. Uses available resources to assist in discharge planning. 12. Plans, prioritizes, and adjusts assignments to accomplish goals and render superior patient care; seeks assistance when needed. 13. Safely operates medical equipment. Serves as a preceptor and assists new staff in the provision of care in order to help them acclimate to the healthcare environment, and a direct patient care role. 14. Adheres to University and unit-level policies and procedures and safeguards University assets. CORE QUALIFICATIONS Education : Graduate from an accredited school of nursing, Bachelor’s degree (BSN) preferred. RNs hired with an Associates (ASN) degree have 2 years to complete the BSN degree. Experience : Minimum 2 years of relevant experience required Certification and Licensing: Valid Florida Registered Nurse License, ACLS, BLS certification from the American Heart Association required.
