The Stem Cell Transplant (SCT) RN Coordinator is responsible for coordination of the evaluation and preparation of patients for Hematopoietic Stem Cell (HSC) transplantation and cellular therapies. The SCT RN Coordinator is responsible for coordinating patient care across multiple areas 'the outpatient clinics, the inpatient nursing unit and the community ' in order to provide continuity of patient care across settings. Problems encountered have a significant level of complexity due to both clinical and logistical issues. Independent thinking and a high degree of problem solving and innovation are required to resolve complex and unanticipated problems, applying appropriate interventions to individual patients and patient problems while performing daily tasks. The SCT RN Coordinator is also expected to serve as a clinical resource for other team members, patients/family/caregivers, referring case managers and community clinicians. Although supervision and direction are provided by the Cellular Therapy Program Manager and physicians, the SCT RN Coordinator has wide latitude in decision making within written organizational and department policies and principles. In addition to independently planning and scheduling their own daily schedule, the coordinator must be able to prioritize clinical issues and case manage a large number of patients in various stages of the transplant/cellular therapy processes. Under general supervision, assumes 24-hour accountability to plan and provide direct professional nursing care for specified patients throughout the entire episode of service delivery as defined by population and unit. MS7 MS 7 is a medical surgical floor with 28 beds where our average patient turnaround and length of stay is between 24 hours to 3 days. We specialize in the care of children with complex Neurology, Cardiology, Pulmonary and Urology conditions as well as other general medical ailments. MS8 and ACGT MS8 is a 27 bed, inpatient floor. Our team works closely with multiple specialty areas to provide compassionate care, innovative treatment options, and novel clinical therapies to children and young adults affected by cancer and blood disorders. Our staff are provided a strong foundation for caring for this population by offering both Hem/Onc Foundations classes and Chemotherapy/Biotherapy classes. Advanced Cellular and Gene Therapy (ACGT) is a six bed unit operating in partnership with MS8 through a shared nursing staff and integrated model of care. Clinical Practice- On a daily basis, demonstrates clinical expertise and uses the nursing process to provide safe, appropriate patient care to a defined patient population according to CCMC and unit policies and standards. Home Health- Hospice Admission Registered Nurse

The Admitting Nurse serves as a member of the Interdisciplinary Team (IDT) and is primarily responsible for admission and resumption of care visits of patients for home care and hospice services, development of the patient plan of care, coordination with the patient's provider and other disciplines for the provision of services, and safe delivery of patient care. Provides a complete physical assessment and history of current and previous illnesses to support eligibility for hospice services. 2. Assesses/reassesses patients and establishes plans of care that are patient-specific. 3. Collaborates with providers, other disciplines, and communicates all changes promptly. 4. Educates patients/caregivers regarding hospice philosophy, disease progression, physical components of care, stages of dying, and other aspects of the POC as necessary. 5. Previous home care/hospice experience preferred. LICENSURE Registered Nurse with a valid (or eligible) Connecticut license required. A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day. CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire. SPECIAL SKILLS Understanding of Medicare certification and CMS data collection tool documentation experience. Strong organizational skills and the ability to work independently. Strong computer skills and the ability to learn new software. Ability to manage a variety of assignments while establishing priorities and ensuring that necessary duties are completed.
