Description The Per Diem Nursing Administrator is a registered professional nurse with progressive administrative experience. S/he supports Nurse Managers, Associate Nursing Directors and senior nursing leadership in the management of clinical units/specialty areas, fulfilling responsibilities for hospital and patient care services on assigned shifts. Responsibilities Patient Care 1. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. 2. Demonstrates proficiency in all aspects of Purposeful Hourly Rounding (PHR) and role models expected behaviors. 3. Ensures the integration of PHR into clinical activities to enhance Relationship Centered Care. 4. Validates staff members round hourly on all patients by direct observation and patient feedback. 5. Assesses staff competency in PHR and provides consistent meaningful feedback with more focused feedback as needed. 6. Oversees that rounding occurs on all patients on shift. 7. Communicates quantitative and qualitative information regarding the patient experience and staff engagement to the staff and the interdisciplinary health care team that leads to the development and implementation of an action plan. 8. Collaborates with the physician dyad and escalates concerns as appropriate. 9. Follows up on all patient concerns in a timely manner and provides service recovery as needed. 10. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. Patient Experience 1. Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions but not limited to: Admission Welcome, Bedside Shift Report, HELP, AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding (PHR), Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. 2. Ensures that staff huddles occur at the start of each shift, identifying high-risk patients who require special attention. 3. Communicates to nursing leadership concerns or issues identified during huddle and throughout shift. 4. Initiates service recovery for patients and family members who have concerns and escalates to the Clinical Nurse Manager/Associate Director of Nursing or designee and Patient Relations as needed. 5. Interprets and communicates patient experience scores and benchmarks and keeps staff informed of progress and areas of opportunity. 6. Empowers staff to use evidence-based practices to increase understanding of patients perceptions of care. 7. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. Quality and Safety 1. Designs, supports and leads a safe, therapeutic and efficient patient-centered care environment for professional practice. 2. Identifies and investigates issues or variances in practice/operations by participating in the RCA process and implements corrective action plans. 3. Fosters a fair and open culture that encourages error and near-miss occurrence reporting. 4. Leads interdisciplinary quality improvement teams using the principles of high reliability. 5. Reviews unit progress, changes, and compliance with quality and safety metrics with clinical nurses and other members of the healthcare team. 6. Responds to escalations of potential safety hazards or gaps from best practice by correcting concerns following institutional chain of command protocol. 7. Monitors institutional information technology infrastructure and corrects gaps in clinical nurse documentation. 8. Leads staff participation in quality and safety initiatives and mandated compliance measures. 9. Demonstrates accountability for all unit quality and safety practices and compliance, and ensures required evaluation of staff competencies is maintained. Operations and administration 1. Supports implementation of the mission, vision, goals and objectives of the department, throughout the clinical service and individual patient care units. 2. Provides administrative and clinical oversight to ensure that all activity (staffing, support services, inter disciplinary collaboration, resources and financial viability) supports the goals of the organization. 3. Works collaboratively with physician dyads and the healthcare team to promote patient care and regulatory standards. 4. Maintains a nursing unit in compliance with all regulatory agency mandates, including the regulations of NYS DOH and The Joint Commission. 5. Oversees patient care delivery, throughput, staffing and processes to ensure efficient and effective operations during assigned shift. 6. Oversees the budget, including management of OT and vacancies, according to MSHS financial goals. 7. Provides input into the management and retention of qualified staff. 8. Maintains patient/employee confidentiality in the management of information. 9. Evaluates nursing staff performance and mentors, coaches, counsels and disciplines staff as needed. 10. Monitors availability and maintenance of nursing equipment and supplies. 11. Conducts regular staff meetings to ensure communication with staff about progress, changes and compliance with all hospital policies and procedures. 12. Encourages, recognizes and rewards staff participation in quality and safety initiatives, compliance and successes. 13. Facilitates organizational change initiatives by using effective strategies. 14. Leads a shared decision making model by establishing a structure of shared governance, just culture and staff engagement in practice councils, departmental meetings and initiatives. 15. Interprets and implements collective bargaining agreements affecting staff. 16. Responds to protest of assignment in a timely manner as detailed in the collective bargaining agreement and discusses the resolution in unit staff meetings. 17. Responds to various hospital emergencies and escalates to appropriate leadership, including acting as incident commander during evening/night shifts and weekends/ holidays. Providing guidance and assistance to nursing personnel in the delivery of patient care. Ensuring continuity of care on other shifts through written and/or verbal communication. Communicating special care needs to the appropriate member of Leadership. Collaborate with the ADON to orient new staff. Evaluate the new nurse on an ongoing basis, write a formal evaluation at the end of the probation period, and make recommendations to the ADON. Monitor, record, and report patients’ medical information, vital signs, symptoms, and changes in condition and escalate care as appropriate according to established policies, procedures, and standards of care. Comply with procedures, regulations, and rules in maintaining medical records. Modify patient treatment plans as indicated by patients’ responses and conditions. Monitor all aspects of patient care, including diet and physical activity. Perform administrative and managerial functions, such as taking responsibility for a unit's staff, and planning. Direct and supervise less-skilled nursing/health care personnel, or supervise a particular unit on one shift. Prepare patients for, and assist with, examinations and treatments. Observe nurses and patients to ensure that proper nursing care is provided. Instruct individuals, and other groups on topics such as health education, disease prevention, and childbirth, and develop health improvement programs. Prepare rooms, sterile instruments, equipment, and supplies, and ensure that stock of supplies is maintained. Administer local, inhalation, intravenous, and other medications as ordered. Provide health care, first aid, immunizations, and assistance in convalescence and rehabilitation. Conduct specified laboratory tests. Direct and coordinate infection control services, advising and consulting with specified personnel about necessary precautions. Provide or arrange for training/instruction of auxiliary personnel. Refer patients to specialized health resources or community agencies. Consult with institutions or associations regarding relevant issues and concerns. Assess patient needs and provide clear education on the management of their care. Engage in nursing research. EMERGENCY RESPONSE Responding to/addressing medical emergencies, such as falls, heart attacks, strokes, burns, and recovering post-operative patients. Evaluate a patient's condition to determine the right course of treatment. When clinically indicated, provide emergency care, such as CPR, AED, shock prevention, bleeding control, and techniques to prevent worsening of spinal damage, etc. Monitor patients' condition and keep it stable while en route to the clinic. Aid in preparing patients for transfer to the emergency units or hospitals. Document the medical treatment that is given to the patient. Keep the equipment clean after use; check and replace damaged and used supplies. Collaborate with the pharmacy to replenish supplies. PHYSICAL DEMANDS Ability to lift fifty (50) pounds. Moving, lifting, or transferring patients may involve lifting up to 100 pounds. Ability to stand for extended periods. Fine motor skills. Visual acuity. Minimum Qualifications A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA). Special Note For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. In addition, must hold or obtain through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA). Department Preferences Licensed Registered Professional Nurse currently registered to practice in New York State. A baccalaureate degree in nursing is preferred. Demonstrated clinical competence and leadership traits (e.g. decision-making ability and skills in problem-solving, communication, priority-setting, and interpersonal relationships) with a minimum of one year of clinical nursing practice as a medical/surgical, behavioral health, or correctional health nursing experience preferred. Demonstrated commitment to continuing professional development through attendance at continuing education programs and participation in professional/organizational committees/councils Basic Life Support Certification is required; ACLS is preferred.
