Documentation in the electronic health record, LifeChart: complete DAP Notes, medical database, care coordination (assessments, injections, etc.) Safely prepare and administer IM injections. Reconcile orders/medications, transcribe medication sheets, and safely pack medications in the group home environment. Admit client to group home environment from outside hospitals/care organizations. Deliver medications/re-payee checks to clients. Timely Escalation-of-Care/Concerns to appropriate parties. Demonstrate cognitive appropriateness to practice: RN’s must know how and know when to perform tasks, manage complex cases and long-term conditions. Demonstrate (shows/does) development of practice and ability for self-reflection and evaluation.
