Maintain and update Section M of the MDS for all residents with wounds. Update Braden scores quarterly or as conditions change. Develop, initiate, and update care plans for alterations in skin integrity. Conduct pressure ulcer investigations for nosocomial sites and track wound trends. Collaborate with the ADNS on interventions to address wound care trends. Communicate promptly with Unit Managers regarding wound-related concerns. Ensure wound care equipment and supplies are readily available. Perform daily assessments of residents pre- and post-nebulizer treatments, documenting findings. Participate in quality assurance audits and data collection for wound statistics. Function independently while upholding facility policies and best practices. Perform other duties as assigned by the Director of Nursing, especially in emergencies. Qualifications Current and valid RN license in the State of New York. Minimum of 6 months of experience in rehabilitative and restorative nursing practices required. 2 years of supervisory experience in a hospital, skilled nursing facility, or related healthcare setting preferred. Strong assessment, documentation, and communication skills. Ability to work both independently and collaboratively in a fast-paced environment.
