Provide immediate response to situations of resident risk such as allegations of abuse, neglect or misappropriation of property. Report incidents timely and to appropriate persons per facility policy. Ensure timely and accurate medication and treatment administration. Communicate to physician and documents changes in resident condition utilizing both objective and subjective nursing assessment. Document and implement changes in nursing and medical plans of care. According to facility policy and procedure, assure that all members of the interdisciplinary team are aware of and understand changes in resident nursing and/or medical plan of care. Writes and reviews care plans identifying the problem/needs of each resident, the care to be given, goals to be accomplished and updating the care plan as needed. Conducts nursing assessments. Reviews resident change in condition report daily to ensure necessary follow up occurs including appropriate interventions are implemented and the family has been notified. Complies with and ensures that staff is in compliance with HIPAA rules and regulations. Initiates disciplinary action as necessary for staff members.
