Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Please note: The 'minimum’ rate and 'maximum’ rate refers to a 'per-visit' rate for regular assignments in this role. Voluntary work opportunities, outside the normal scope of duty, may pay an alternate ‘per-visit' rate, depending on multiple factors (region, type of visit, discipline, etc.), and may be offered on an occasional, as-needed basis. There is an hourly rate paid for certain administrative tasks (orientation, training, meetings, etc.), which is $30.00, following completion of new hire orientation. Job Responsibility Conducts pre-admission screening and initial nursing health assessment on new patients; assesses body systems (cardiac, vascular, neurological, and gastrointestinal, etc.); develops nursing plan of care and goals based on patients' needs. Provides nursing care to patients to include but not limited to supervising diet, medication, activity level of client, supervising safety in the home, and activities of daily living. Evaluates the provision of nursing care to patients, including but not limited to Evaluation of physical therapy, occupational therapy, speech therapy, nutrition counseling, and respiratory therapy, Contracting agency nurses. Trains and evaluates home health aides, homemakers, and personal care workers in the patient’s home, recommending personnel changes to the administration of Hospice as is necessary. Determines the need for, and recommends the rental or purchase of medical equipment and/or supplies for patients. Provides ongoing evaluation of equipment for defects, repairs, or additional needs. Recommends to the Hospice IDG (Interdisciplinary Group) when physical therapy, occupational therapy, speech therapy, nutritional counseling and respiratory therapy are warranted, as per his/her clinical assessment; requests physician written orders for changes in the plan of care as is appropriate; reports to and consults regarding patient status; participates in on-going orientation and in-service training offered both within and outside the Hospice. Participates in on-call coverage system on a rotating basis; serves as a liaison in maintaining the Hospice patient's Plan of Care while inpatient; refers major or recurrent problems to the Hospice supervising nurse. Participates in the ongoing Quality Improvement Program. Performs related duties as required. You will: Consult with internal client groups to guide quality and patient safety initiatives Develop, implement, and enhance programs that uphold the highest standards of patient care Research, collect, and analyze internal and external data to inform decisions Act as a subject matter expert in quality, patient safety, significant event management, and operational priorities aligned with NYP’s strategic goals This position operates Monday–Friday (4 days per week; 7A - 5:15P) and is primarily based at NYP Queens in Flushing, NY . Travel may be required to multiple NYP Hospitals depending on assignment and to the Corporate office in Midtown Manhattan for departmental meetings and events. Required Criteria New York State RN License Master of Science in Nursing required BLS from American Heart Association ACLS within 90 days of hire NRP within 90 days of hire Certification in Inpatient Obstetrical Nursing NCC Certification in Fetal Monitoring Minimum 5 years of clinical experience, of which at least 2 years are related to staff education. Must have a working knowledge of Leopold Maneuvers and AWOHNN Standards Must have a working knowledge of Triage, Labor Room, Operating Room and Recovery Room functions Ability to interpret normal and abnormal fetal heart patterns Advanced knowledge and skills in the recognition and management of obstetrical emergencies within a multidisciplinary care environment Join a healthcare system where employee engagement is at an all-time high.
