Department/Unit: Neurosciences & Epilepsy Monitoring Unit (EMU) - D5N Work Shift: Day (United States of America) Salary Range: $83,200.00 - $93,184.00 The Clinical Nurse assesses, coordinates, plans, directs, implements and manages the needs of assigned patients throughout the episode of care with a focus on progress toward discharge, including during transitions within the acute care stay. The Clinical Nurse is responsible for the delivery and coordination of patient care provided by the caregiving team. S/he assures that competent, compassionate patient care is uniformly provided to customers including referring facilities, hospital staff and patients in the inpatient, ambulatory, and community settings. D5N is a 21 bed neurosciences unit with one level of care. We take patients with ischemic and hemorrhagic strokes, brain tumors, aneurysms, seizures, traumatic brain injuries, neuromuscular diseases and other diagnoses involving the neurologic system. The floor beds consist of patients requiring every 4-hour monitoring. Additionally, there are 7 Epilepsy Monitoring Unit (EMU) beds embedded in the 21 neuro floor beds. These patients are seizure disorder patients admitted electively with their anti-convulsant medications on hold and hard wired to continuous EEG. This allows the physicians to identify the area of the brain where the seizures are occurring. Many of these patients become candidates for brain mapping with surgically placed grids to further pinpoint the area causing the seizures. This mapping allows the surgeon to determine if the patient would be a candidate for surgical removal of parts of the brain to stop seizure activity and still allow the patient to have functionality. The nurse-to-patient ratio for floor patients is typically 1:5. Responsibilities Job Duties / Responsibilities The Director of Nursing (DON) is responsible for managing all aspects of our nursing staff to ensure our patients in residence receive quality care that protects their rights, dignity and health. Duties and responsibilities may include and are not limited to: Collaborating with medical, mental health and correctional staff to enhance quality patient care while maintaining a safe and secure work environment Leading Continuous Quality Improvement (CQI) activities Managing nursing operations, resources, budgets, and staffing schedules, hiring and training of new staff, and conducting performance evaluations Ensuring staff adhere to current and new policies and procedures and company Code of Conduct Effectively communicate with healthcare staff and correctional staff to coordinate patient care with providers outside of the facility Monitoring compliance with site-specific and company policies and procedures, taking appropriate corrective action, and ensuring compliance with ACA, NCCHC standards Ensuring all heath care related equipment and supplies, including pharmaceuticals and over the counter medications, are appropriately stocked and available at all times Supporting staff in developing and maintaining problem-solving and nursing diagnoses competency Conducting daily clinical rounds to all designated areas of responsibility to observe and review documentation, orders, and patient care plans, and ensuring objectives are understood and practiced by the nursing staff Conducting educational classes, in-service training and training staff on new procedures/policies Mitigating risks by proactively addressing potential hazards, including dangerous work conditions, health and safety concerns and reporting issues to company leadership and facility management Participating in all monthly, quarterly, and annual meetings as assigned and required in accordance with contract compliance Qualifications Experience / Requirements / Qualifications Holds a current, unrestricted license as a Registered Nurse within Florida Must possess an active BLS/CPR Certification (AHA preferred) Three years of clinical and progressive management experience in a government or private organization related to comprehensive medical, dental, and mental healthcare services within a correctional healthcare or acute care setting Ability to manage complex staffing matrices and clinical supplies Excellent communication skills with proven experience leading, managing, and coaching medical staff through complex Human Resource issues and challenges. Knowledge of nursing practice principles and techniques, occupational health and safety standards, infectious disease management, applicable health care laws, regulations, and procedures for direct health care services. Ability to maintain confidentiality of all proprietary and/or confidential information Integrity, professionalism and ability to adhere to a Code of Conduct and comply with all facility(s) correctional healthcare policies, procedures, and legal requirements Must pass the facility’s criminal background check and drug screening Physical Capabilities: Must have the ability to stand for long periods, lift and move patients (up to 50 pounds), bend, squat, reach, push, pull, walk significant distances, and perform physically demanding tasks like CPR, all while maintaining good balance and endurance throughout a shift. Description The (Per Diem) Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. 3. Integrates relevant assessment and intervention skills in the delivery of nursing care. 4. Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions including, but not limited to, Admission Welcome, Bedside shift Report, HELP, AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. 2. Establishes and maintains a therapeutic relationship with the patient and family. 3. Uses evidence-based practices to increase understanding of patients perceptions of care. 4. Initiates service recovery for patients and family members who have concerns and escalates to Clinical Nurse Manager/Assistant Nurse Manager/Nursing Shift Manager or Patient Relations as needed. 5. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. 6. Supports staff use of evidence-based practices to increase understanding of patients perceptions of care. 7. Conducts Purposeful Hourly Rounding (PHR) on assigned patients addressing the 4 Ps. Quality and Safety 1. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. 2. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. 3. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. 4. Engages in formal and informal peer and institutional review processes. 5. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. 6. Contributes to quality and safety practices and required compliance measures. 7. Implements principles of high reliability to identify and avoid high risk behaviors to provide a safe environment for patients. 8. Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol. 9.
