Overview: In collaboration with patients, families (as defined by the patient) and staff across all disciplines and departments is responsible for planning, organizing and directing the clinical support staff assigned to a unit in a community healthcare center. Responsibilities: Essential Duties: Performs Charge Nurse Duties in absence of Charge Nurse. Oversee assigned staff, including but not limited to, establishing work assignments and oversee flow of unit along with identifying staffing needs. Assess the physical, social and emotional needs of patients and families in clinics. Teach and counsel patients and families concerning health assessment and care including prevention of disease, disability, hygiene, and health maintenance. Participate in the identification of patient needs for referral to resources that facilitate the continuity of care in conjunction with care management team. Provide direct nursing care to patients and distribute medications and treatments according to accepted standards of care. Complete all direct and indirect care documentation in timely manner and ensure patient records are current and complete. Participates in daily patient care (unit) huddles, in some case ensuring that huddles are taking place along with weekly clinic staff informational huddles. Maintains all licensure and/or certification requirements for continuing education and best practices. Identifies, in collaboration with Charge Nurse, supports and participates in quality strategies in collaboration from Charge Nurse to evaluate compliance with evidence-based guidelines, standards, and to identify opportunities to improve patient outcomes. Ensure all tasks provided and associated with patient care, patient administrative processes and related duties comply with all regulatory and accreditation standards including The Joint Commission and CommUnityCare Standard Operating Procedures and CommUnityCare Policies and Procedures. Act as the site lead in the absence of the Nurse Manager Operations and/or Practice Leader. Daily/weekly quality assurance checklist to include delegation of tasks as appropriate to staff (Rider report, class D, McKesson, CLIA, AED, equipment checks) Ensures state vaccine (VFC/ASN) policies are being followed. Attends all clinic codes. Patient triage as needed. Performs periodic chart audits. De-escalation of crisis situations by seeking guidance of supportive personnel. Perform other duties as assigned. Knowledge/Skills/Abilities: High level of skill at building relationships and providing excellent customer and patient care. High degree of knowledge and competency in the practice of nursing and documentation requirements Requisite skills and ability to perform assigned nursing tasks. Qualifications: MINIMUM EDUCATION: Graduation from an accredited school of nursing PREFERRED EDUCATION: Bachelor of Science in Nursing (BSN) preferred MINIMUM EXPERIENCE: Graduate from accredited school of nursing. Demonstrate success in providing clinical care and nursing support within an organization of comparable pace and complexity. Experience managing difficult patient/customer situations. Skilled Nursing & Clinical Care Provide skilled nursing interventions according to resident needs and the RN's scope of practice. Perform treatments that may include wound care, catheter care, enteral feeding support, respiratory care, and other clinical procedures as ordered and appropriate. Monitor residents with acute and chronic health conditions. Support pain and symptom management while promoting resident comfort and dignity. Respond appropriately to changes in condition, emergencies, and other clinical situations. Collaborate with physicians and advanced practice providers regarding resident treatment and clinical needs. Clinical Leadership & Nursing Team Support Provide clinical guidance and direction to LPNs, CNAs, CMTs, and other nursing team members as appropriate. Delegate nursing responsibilities according to applicable regulations, resident needs, and team-member scope of practice. Support nursing assistants and other caregivers in delivering safe, compassionate resident care. Serve as a clinical resource for nursing team members. Promote teamwork, accountability, professionalism, and effective communication. Assist with orientation, mentoring, and education of nursing employees as assigned. Interdisciplinary Collaboration Work closely with Nursing, Therapy, Social Services, Dietary, Activities, physicians, and other healthcare professionals to coordinate resident care. Quality, Safety & Regulatory Compliance Follow applicable federal and state regulations, professional nursing standards, and facility policies. Support infection-prevention and resident-safety practices. Participate in quality assurance and performance improvement initiatives as appropriate. Assist with incident investigations, clinical reviews, and corrective actions when assigned. Help maintain continuous survey readiness through accurate documentation and consistent nursing practices. Identify and report potential resident-safety or clinical concerns promptly. Perform other duties as assigned. What We're Looking For Graduate of an accredited nursing program. Current, unrestricted Registered Nurse (RN) license in the state Missouri. 1–2 years of nursing experience preferred, particularly in long-term care, skilled nursing, rehabilitation, senior living, or geriatrics. New graduate RNs may be considered based on community needs and applicable requirements. Strong nursing assessment and clinical decision-making skills. Knowledge of geriatric nursing, chronic disease management, medication administration, and resident care practices. Ability to recognize and respond appropriately to changes in resident condition. Caregiver Summary: The Caregiver is a member of the home care team who works under the supervision of a Care Manager to provide personal care and support services that meet each client’s individual needs.
