Position Summary The Licensed Practical Nurse (LPN) provides direct nursing care under the supervision of a Registered Nurse (RN) or Nursing Supervisor. The LPN is responsible for administering medications and treatments, monitoring resident health, documenting care, and supporting residents' physical and emotional well-being while ensuring compliance with all federal, state, and facility regulations. Essential Responsibilities Provide direct nursing care to residents in accordance with individualized care plans. Administer prescribed medications, injections, treatments, and wound care accurately and safely. Monitor residents for changes in condition and promptly report findings to the Registered Nurse or physician. Complete accurate and timely documentation in the electronic medical record (EMR). Qualifications Required: Current New York State Licensed Practical Nurse (LPN) License. Graduate of an accredited Practical Nursing Program. Current CPR/BLS Certification. Strong communication, organizational, and interpersonal skills. Ability to work effectively in a fast-paced skilled nursing environment. Preferred: Previous experience in long-term care, rehabilitation, or skilled nursing. Experience with electronic medical records (EMR).
